To unlock the Organ Transport Box, examine it in your inventory, turn it over, remove the rear panel, then set the controls to HAND → far-right switch middle → switches 5 and 2 down → AUTO. It is in the Garage at the Rhodes Hill Care Center’s Patient Center, and opening it gives you the Artificial Lungs.
Where to find the Organ Transport Box
Look for the box in the Garage on the Patient Center ground floor, in the southwestern corner. The route described by Vandal’s walkthrough goes through the Level 1 security door in the southwest, after the Janitor’s Office. Nintendo Life’s guide also places it in the Care Center Garage.
How to open the Organ Transport Box
- Pick up the box and examine it from your inventory.
- Turn it over and remove the rear panel to reveal the controls.
- Turn the large dial to HAND.
- Move the far-right switch to its middle position.
- Lower the two yellow switches: controls 5 and 2.
- Turn the large dial back to AUTO. The box opens.
The far-right switch is the one that goes to the middle; do not confuse it with the two switches you lower. PowerPyx’s puzzle guide corroborates the sequence and says puzzle solutions are the same across difficulties.
Where to find the in-game instructions
The clue is in the Leatherbound Notebook in the Lead Researcher’s Office. Find it inside the puzzle box there; the document is titled “Unlocking the Organ Transport Box.” However, you do not need to find the notebook before opening the transport box. PC Gamer notes that the box can be opened whenever you reach it, while Evil Resource identifies the notebook document.

What the box contains
Inside is the Artificial Lungs. Use them on the Organless Corpse in the Examination Room as part of the Care Center progression. The lungs are also needed eventually to obtain a Level 2 Wristband, which opens additional areas, according to Nintendo Life and Vandal.
