What changes Arthur’s ending?
In the Chapter 6 mission “Red Dead Redemption,” you choose whether Arthur helps John escape or returns for the money Dutch hid. Combined with Arthur’s honor level, that choice produces four ending variants. “Good ending” is a common label for the more redemptive version, not a survival route or a confirmed official canon.

The ending guide from RDR2.org describes honor-linked differences in Arthur’s reaction to his tuberculosis diagnosis and in the animal visions that follow: an honorable Arthur sees a deer, while a dishonorable Arthur sees a wolf. These are changes in presentation and tone; they do not cure him or avert the ending. Read the RDR2.org ending guide.
Does helping John save Arthur?
No. Helping John determines which final branch you see, but Arthur still dies. Roger Clark, who played Arthur, described the structure as four endings: high- and lower-honor versions of helping John, and high- and lower-honor versions of returning for the money. He said none is a happy ending for Arthur. That is Clark’s characterization, not an official declaration of which branch is canon. GamesRadar+ reported Clark’s comments.

Can anything else change his fate?
Reaching maximum horse bond unlocks a special scene during the final mission, according to the RDR2.org guide. It adds an optional goodbye, but it does not change what happens to Arthur.

Story Mode continues into an epilogue centered on John Marston. Arthur’s death is fixed, but the game carries on beyond his final chapter. Rockstar’s official game page describes the story as Arthur weighing his ideals against his loyalty to the gang and lists Story Mode as part of Red Dead Redemption 2. See Rockstar’s game page.
