A guide places its drop on an Omenkiller outside the Lower Capital Church Site of Grace in Leyndell, Royal Capital. Whether you can still obtain it after Leyndell becomes the Ashen Capital is not established by the available references.
What the Omensmirk Mask does
The Omensmirk Mask is head armor worn by Omenkillers. Its item description says it increases Strength, and Eldenpedia lists the effect as +2 Strength. Eldenpedia also lists its weight as 3.0.
The in-game description reproduced by Eldenpedia reads: “Mask with long, hideously twisted horns worn by the Omenkillers. Increases strength. Bears the smirking face of an elder, twisted in wicked delight. This visage is carved in the image of the evil spirits that haunt the Omen in their nightmares.” Eldenpedia’s item reference is a community-maintained source, not an official game-data page.

Poise figures differ between references
Published item references do not agree on poise: Eldenpedia lists 6, while Game8 lists 2. Tarnished.app also lists 6 in its overview and notes a poise update for patch 1.07, but the returned information does not specify the earlier value. Without version-specific game data, there is no sound basis here to label one figure as the current value.
