To complete Tribal Medicine in Path of Exile 2, defeat the Great White One on Whakapanu Island, collect the Shark Fin, and give it to Kaimana. She is in Ngakanu, or later on the Karui boat in Kingsmarch. You then choose one of two blessings; the quest guide says the choice cannot be changed.
How to get the Shark Fin
- Travel to Whakapanu Island and defeat the Great White One.
- Pick up the Shark Fin it drops. The fin is the item you need to advance the quest.
Tribal Medicine can begin when you pick up the fin, or when you visit Ngakanu and speak to Kaimana. If you accepted the quest before finding the fin, return to Whakapanu Island and defeat the Great White One.
Where to find Kaimana and hand in the fin
Take the Shark Fin to Kaimana in Ngakanu and speak to her to hand it over. If story progress has moved her from Ngakanu, look for her on the Karui boat in Kingsmarch instead. Once you give her the fin, choose one of the two rewards.
Which Tribal Medicine reward should you choose?
The quest page lists the reward choice as unchangeable. Compare the blessings with your character’s defenses before selecting one; neither is established as universally best.

Why the quest may say “Find a use for the Shark Fin”
If you acquired the Shark Fin before accepting Tribal Medicine, the quest can show the objective “Find a use for the Shark Fin.” Take the fin to Kaimana in Ngakanu—or to the Karui boat in Kingsmarch if she has moved—to complete that objective.

PoE2DB’s Tribal Medicine entry also identifies the Shark Fin and Kaimana as central to the quest. Game8’s guide, last updated December 18, 2025, confirms the Great White One and Shark Fin steps, but an older Game8 result describes different rewards. For the current reward choice, use the two blessings listed above rather than that older description.
