Spoilers for the ending. Caz McLeary blows up the Beira D while he is still aboard, trying to stop the entity from reaching the mainland. The game then cuts to a surreal image of Caz beside his wife, Suze, before he opens a door onto the ocean. The explosion is the ending’s clear plot event; the final vision does not confirm whether Caz has died, reached an afterlife, or been absorbed by the entity.
What happens in the ending?
Still Wakes the Deep is set in 1975 aboard the Beira D, a North Sea oil rig off Scotland. After the rig is overwhelmed by an otherworldly entity and escape efforts fail, Caz concludes that it cannot be allowed to reach the mainland. He sets off an explosion, sacrificing the rig with himself still aboard. The official Steam listing describes the game’s setting as a disaster aboard the Beira D; the ending account from The Escapist reports that Caz blows up the rig to stop the entity.
What does the final scene mean?
After the explosion, Caz passes through a glowing, star-filled tunnel. He appears to wake beside Suze, then opens a door and finds the ocean beyond it. That image resists a simple reading of Caz having returned home: the sea is still present, linking the vision to the catastrophe on the rig.
The scene can be read as Caz dying, but that is not confirmed. The ending account also raises the possibility of an afterlife or Caz’s absorption into the entity. The game leaves his exact fate unresolved, so those explanations are interpretations rather than established canon.

Is there an alternate ending?
The ending account describes one ending: Caz blows up the Beira D with himself aboard. The available sources do not establish player-selected alternate endings or a choice that changes his fate.
Does Siren’s Rest explain what happened to Caz?
No definitive answer about Caz’s fate is established by the later expansion’s setup. Siren’s Rest takes place in 1986, eleven years after the original, and follows saturation diver Mhairi as she explores the Beira D and investigates what happened there. Its premise continues the story in the same setting, but does not settle whether Caz died, entered an afterlife, or became part of the entity. See the expansion writer’s account on Xbox Wire.

Why does the ending feel unresolved?
The ending’s ambiguity fits the game’s combination of supernatural horror and grounded human drama. In an Xbox Wire discussion of the original game’s inspirations, lead designer Rob McLachlan described its elevator pitch as “The Thing on an oil rig.” The developers also discussed isolation, paranoia, fear, and human relationships as central concerns. That context helps explain the balance between a concrete sacrifice and an unexplained final vision, but it does not provide a definitive in-world explanation for the entity or Caz’s fate. Read the developers’ comments in Xbox Wire’s 2023 interview.
