The 4th Survivor is a bonus mode in Resident Evil 2 starring HUNK, the only member of an Umbrella Security Service team to survive a mission at NEST. You play through his escape toward the Raccoon City Police Department roof. Capcom says the mode unlocks after you complete the main story.
What happens in The 4th Survivor?
In Capcom’s account, U.S.S. Alpha Team enters NEST to recover a G-Virus sample from William Birkin. Birkin mutates and kills the rest of the team, leaving HUNK to fight his way toward the police station roof and escape by helicopter. The mode puts you in HUNK’s role during that escape; it is a bonus scenario, not another main-campaign chapter. Capcom’s account of HUNK and the scenario also notes that The 4th Survivor appeared in the original 1998 Resident Evil 2.

How do you unlock it?
Capcom’s stated requirement is to complete the main story. Its description does not specify a more exact sequence of character campaigns or runs, so there is no basis here for adding one.
Is The 4th Survivor the same as The Ghost Survivors?
No. The 4th Survivor is HUNK’s escape scenario. The Ghost Survivors is a separate free update built around alternate scenarios; PlayStation Blog announced it for February 15, 2019, and described The 4th Survivor as an already familiar additional mode. PlayStation Blog’s announcement distinguishes the update from HUNK’s scenario.
