Return to every altar used during ring assembly and verify its final state. Fully completed altars remain visually inert, with no interact prompt, no glow, and no ambient audio.
If an altar still hums, pulses, or shows an interaction outline, it means a fragment was inserted out of order or a delayed chest failed to spawn. This is the most common reason players miss a Precious Chest even after finishing the ring.
Also check for terrain that looks “done” but still allows climbing or clipping. Invisible walls disappearing after ring completion often guard a last reward tucked just out of view.
