How to Return to Running After a Hamstring Injury

There is no universal number of days to wait after a hamstring injury. Use symptoms, strength, functional capacity and your response to loading to guide a gradual return.

There is no reliable calendar rule for when every runner can resume running after a hamstring injury. The best-supported approach is to return gradually, using symptoms, strength, functional capacity and your response to prior loading—not a fixed number of days—as readiness checks. An easy controlled run is an earlier milestone than sprinting, unrestricted training or competition.

How to decide when to try running

The 2023 London International Consensus on hamstring injuries favors criteria-based return to running over a fixed timetable, while noting that clinicians do not agree on one set of criteria. The involved muscle, injury characteristics, running demands and progress through rehabilitation all matter. A date alone cannot account for those differences.

Before trying a run, consider whether symptoms and function are improving and whether the hamstring has tolerated the loading you have already done. Strength and functional capacity are part of that picture; a clinician can help assess them and tailor the decision to your injury and goals.

Use pain as information, not a universal score

Pain is relevant, but the consensus panel did not establish a universal pain cutoff for running. It agreed that mild pain may be acceptable during some rehabilitation activity when the athlete and rehabilitation team agree on a threshold. That is not a blanket instruction to run through pain: it is a reason to make the decision in context with a clinician, symptoms and the response to loading.

The panel prefers pain-free running before sprinting. If pain increases, symptoms worsen, or your response after a running exposure is concerning, do not simply push ahead with the next increase; seek individualized guidance about whether to modify or pause progression.

Build capacity before adding speed

Hamstring rehabilitation progresses toward the demands the runner needs to tolerate. Later-stage work commonly aims to restore strength at longer muscle lengths and eccentric strength. Running and sprinting are also important rehabilitation exposures, but they should be introduced progressively rather than treated as an all-or-nothing return.

Use each stage to assess the response before increasing demand. The consensus supports using symptoms, strength and the response to previous loading to guide progression and dosage. It does not establish one safe weekly mileage increase, pace schedule or timetable that applies to everyone.

Separate an easy run from sprinting and sport

Returning to a controlled easy run does not mean the hamstring is ready for maximal speed or competition. Increase speed and overall running demand progressively, with sprinting treated as a later and more demanding exposure. The consensus report found no agreed criteria for safe return to sprinting, so readiness should be assessed for the individual rather than inferred from a particular day or test alone.

Return to sport is a further decision. The consensus emphasizes pain-free sprinting and completion of unrestricted training that matches the sport’s required volume, speed and intensity. The decision should be shared by the athlete and care team, including a discussion of risk.

Why there is no single test or timetable

The London International Consensus panel set a 70% agreement threshold for consensus. Its 2023 report recorded a final survey response from 99 of 112 experts (88.2%). Agreement was reached on several rehabilitation topics, but not on a definitive flexibility or strength benchmark for clearance: agreement on flexibility benchmarks was 40%, and on strength benchmarks 66.1%, both below the threshold. These are expert-panel agreement figures, not recovery rates or estimates of an individual’s reinjury risk.

The report also identifies unresolved questions about tolerated pain, safe running dosage and how the specific hamstring muscle involved should affect progression. A 2018 review describes phased rehabilitation, moving from symptom management and slower neuromuscular control toward faster work and eccentric strengthening in lengthened positions, while also noting the lack of universal return-to-sport criteria. Neither source supports turning a general phase model into a guaranteed schedule for every runner.

When to get an individualized assessment

Consider assessment by a sports physical therapist or sports medicine clinician if the diagnosis is uncertain, symptoms are concerning, progress is not as expected, or you need help deciding how to resume running. A prior hamstring strain is a recognized reinjury risk factor in the 2022 clinical practice guideline. The guideline also advises caution when a runner has not completed an appropriately progressed, comprehensive functional exercise program that includes eccentric training. A clinician can help determine what rehabilitation remains relevant before you increase running demands.
