Spoilers for the Bloody Baron questline: The Baron’s family fate is primarily determined by whether Geralt kills or frees the spirit beneath the Whispering Hillock. Killing it leaves Downwarren standing but ends with Anna alive and mentally broken; freeing it in the ordinary quest sequence saves the orphans from the Crones, destroys Downwarren, and leads to Anna’s death and the Baron’s suicide. Neither branch gives the family a wholly happy ending.
Which choice determines the Baron’s ending?
The decisive choice happens during The Whispering Hillock, before Return to Crookback Bog resolves the Baron’s family story. The later return quest reflects whether Geralt killed or freed the tree spirit; visiting Tamara in Oxenfurt can alter some dialogue, but it does not replace that main choice. The related quest lines are Family Matters and Ladies of the Wood. The optional Return to Crookback Bog quest becomes available after those lines conclude. If you want to choose with the consequences in mind, compare the two branches below before resolving the Hillock encounter.
What happens if you kill the Whispering Hillock spirit?
- Anna: She survives, but is mentally broken.
- The Baron: He takes Anna away to seek help in the Blue Mountains.
- Downwarren: The village remains intact.
- The orphans: The Crones take them.

What happens if you free the spirit?
In the ordinary quest sequence, freeing the spirit lets the children escape the Crones, but Downwarren is devastated. Anna is cursed; after the curse is lifted, she dies, and the Baron hangs himself.
- Anna and the Baron: Anna dies after the curse is lifted, and the Baron dies by suicide.
- Downwarren: The village is destroyed.
- The orphans: They escape the Crones.

Does the doll save Anna?
No. The hollyhock-flower doll is identified as the one associated with lifting Anna’s curse, but lifting the curse does not save her: she dies, and the Baron hangs himself. The doll therefore changes how the curse is addressed, not the branch’s tragic family outcome.

Can you save Anna and the Baron?
There is no wholly happy ending for the Baron’s family in the two well-supported principal branches. Killing the spirit leaves Anna alive but mentally broken and sends the Baron away with her to seek help; freeing it leads to Anna’s death and the Baron’s suicide. The tradeoff is whether you prioritize the orphans’ escape or avoiding Downwarren’s destruction and Anna’s death.
Does quest order change the outcome?
Walkthrough and community references report an order-sensitive variant if Geralt frees the spirit before consulting the Downwarren elder or before the expected quest sequence. The reported conditions are not consistent enough to promise a precise route or guaranteed result. The principal outcomes above describe the ordinary sequence. Earlier choices, including visiting Tamara in Oxenfurt, can change dialogue and timing without changing the main Hillock-based fates.
