The five-hour estimate is for completing the story, not for doing everything in each mission. GameSpot reports 11 intel items, one per mission; searching for them can add time. Insider Gaming also notes that collecting intel or pursuing exclusive camos can extend a run. Neither source gives a reliable numerical total for a completionist playthrough.
Does campaign length include Endgame?
No. Endgame unlocks after the first 11 co-op campaign missions, so it sits beyond the linear story counted in the estimates above. Xbox Wire Editor Mike Nelson described it as JSOC exploring Avalon, attacking Guild strongholds, and dealing with exposure zones. Call of Duty characterizes Endgame as a replayable campaign mode; the consulted sources do not establish a fixed time to complete it.

Endgame is designed for 1–32 players, in squads of up to four, according to Xbox Wire. It continues activity in Avalon rather than adding a single twelfth story mission. You can finish the 11-mission story and then keep playing Endgame without folding its open-ended playtime into the campaign estimate.
