When suspend’s final time counter is removed, its triggered ability lets you cast the card without paying its mana cost as that ability resolves. “Free” means you skip the mana cost—not every cost or restriction. The exact wording on the card matters: some say you must cast the card if able; others say you may.
When do you cast a suspended card?
You cast it while the suspend triggered ability is resolving, not later. The normal timing permissions based on card type are ignored, so a suspended sorcery can be cast during the resolution even when you could not ordinarily cast a sorcery. Which costs do you pay?
You do not pay the mana cost
The suspend instruction supplies an alternative way to cast the card: without paying its mana cost. You cannot choose a different alternative cost instead, such as overload or evoke.
Additional costs still apply
You may pay optional additional costs, such as kicker. If casting the card requires a mandatory additional cost, you must pay it to cast the card. If that cost requires mana and you do not produce or pay it, you do not cast the card.

X is zero
If the card’s mana cost includes {X}, you must choose 0 for X when casting it without paying its mana cost. Its mana value is still determined by its printed mana cost, not by the amount you paid. These cost rules are explained in the Commander Masters Release Notes.
Must you cast the card?
For the general suspend wording, you must cast the card when the trigger resolves if you are able. But a card can instead make the cast optional with “may”; follow that card’s wording. For example, Suspended Sentence has “Suspend 3—{1}{B}” and says you may cast it without paying its mana cost when its final time counter is removed. Its ruling says that if you do not cast it, or cannot, it stays exiled without counters and is no longer suspended. See the Duskmourn: House of Horror Release Notes.

What if you cannot cast it?
If the trigger is countered, or the card cannot legally be cast as the trigger resolves—for example, because a spell requires targets and there are no legal choices—the card remains in exile without time counters and is no longer suspended. The same result applies when a card’s optional wording lets you decline to cast it.
