No—not in vanilla Skyrim. You can’t add a second enchantment to an item that is already enchanted. The Extra Effect perk lets you choose two effects together when enchanting an eligible unenchanted item; it does not upgrade an item later. You may be able to replace an existing enchantment instead, depending on the item.
Adding a second enchantment is different from replacing one
Skyrim’s enchanting system can create or overwrite enchantments, but overwriting is not the same as adding another effect to an already enchanted item. Bethesda’s general explanation describes enchantments as effects that can be added or overwritten on items, while the game’s enchanting workflow creates a magic item by combining an item, an effect, and a soul gem. That workflow does not provide a way to append a second enchantment to an item that already has one. Bethesda Support’s overview of enchantments and the Skyrim manual describe those basics.
What Extra Effect does—and doesn’t do
Extra Effect lets you select two enchantment effects in the same enchanting action for an eligible unenchanted item. It does not let you return to an already enchanted item and add a second effect afterward. Both effects have to be chosen together when you make the new enchanted item. This specific perk behavior is described in a community Q&A; it is not an official Bethesda explanation.
Can you replace an enchantment that is already on an item?
Sometimes. Bethesda Support says any player-crafted enchantment can be replaced, but some other enchantments cannot. Look for a lock icon above the enchantment: Bethesda says it marks an enchantment that cannot be replaced. Bethesda also states that item traits are permanent. How to use an effect from an enchanted item
If you want an enchantment effect from a found item, disenchanting is the route to learn an eligible effect. The source item is destroyed in the process, so this does not preserve it for a later upgrade. You can then use the learned effect when creating another item at an enchanting station, using the item, effect, and a soul gem described in the official Skyrim manual.

Mods can change this behavior. For example, the Nexus listing for Enchant Uniques describes adding effects to some already enchanted unique items. That is a mod-specific exception, not a vanilla feature; which items work and whether the mod is compatible depend on the mod and the player’s setup. Check its current description and supported game edition before installing or relying on it.
