Borderlands 4 has three difficulty settings when you create a character: Easy, Normal, and Hard. Hard makes combat tougher and officially offers increased Cash and loot quality; Easy reduces incoming damage. Co-op players can set their challenge individually. True Mode and Ultimate Vault Hunter Mode (UVHM) are separate post-campaign challenges, not additional campaign difficulty settings.
What are the Borderlands 4 difficulty levels?
The three campaign options are Easy, Normal, and Hard. Their descriptions and effects are qualitative: the official guidance does not give numeric damage, Cash, or loot multipliers.

These descriptions come from Borderlands Support’s accessibility information. It does not quantify the increases on Hard, so there is no supported percentage to use when comparing its rewards or incoming damage.
When and where do you choose a difficulty?
The initial difficulty is selected during character creation. The official accessibility information describes the setting as a character choice, and does not establish that you can change Easy, Normal, or Hard later through a general in-game difficulty toggle.

Can co-op players use different difficulties?
Yes. Borderlands 4 difficulty scales individually in co-op, so players in one party can tune encounters to their own preferred challenge instead of sharing one party-wide setting. Gearbox explains this approach in its co-op overview on Xbox Wire. If one player wants a more forgiving run and another wants more resistance, agree on expectations before starting together.
What is True Mode?
True Mode is an optional post-campaign challenge setting, unlocked after completing the campaign. It is found in Session Settings and works independently of UVHM. In December 2025 update notes, Gearbox said: “True Mode scales up all enemies as if you were playing in a 4-player party, regardless of your current party size.” Enemies are tougher and more numerous, with more Badass elites. How does UVHM differ from True Mode?
Ultimate Vault Hunter Mode (UVHM) is a separate post-game challenge progression, not another name for Easy, Normal, Hard, or True Mode. The original 2K endgame announcement described five unlockable levels advanced through curated mission challenges rather than replaying the campaign. December 2025 support notes later confirm that UVHM 6 was added. Those sources do not establish the maximum rank available on October 8, 2026, so UVHM 6 should not be treated as a confirmed current cap.

Which difficulty should you choose?
- Choose Easy if a forgiving story playthrough matters most.
- Choose Normal if you want the game’s balanced option.
- Choose Hard if you want a tougher fight, greater emphasis on elemental affinities, and the documented Cash and loot-quality increases.
- In co-op, decide individually. Each player can tune their challenge; the group does not have to assume everyone wants the same difficulty.
- After the campaign, consider True Mode or UVHM separately. They are optional post-game challenge systems, and True Mode is independent of UVHM.
