How to get the Escape From the Hospital achievement in Genshin Impact
- Go to Dar al-Shifa in Sumeru and locate the cave entrance near the abandoned hospital. The entrance is behind a broken fence, with loose rocks blocking the opening. See the GameWith route guide.
- Clear the rubble and enter the cave. Break the boulders or loose-rock barriers that block the way as you proceed. Claymore attacks or Geo damage can break the route-blocking boulders, according to Pro Game Guides.
- At the branching clearing, take the passage that continues toward the cave exit. Enemies and chest detours are optional if your goal is only to reach the exit.
- Keep clearing barriers along the route until the exit sequence plays.
- Outside the cave, interact with the nearby Severely Eroded Hoe. This is the final trigger for the achievement.
What the achievement is called now
GameWith’s page, updated October 5, 2026, calls the achievement One Flew Over the Sick Men’s Rest and identifies Escape From The Hospital as its former name. The name change is reported by that guide; the available official-site search did not confirm it.
Do you need to complete an Archon Quest first?
The available guides do not agree on a prerequisite. Pro Game Guides says to complete Chapter III: Act IV, “King Deshret and the Three Magi,” until Dar al-Shifa is freely explorable. GameWith’s updated route does not list that requirement. If you cannot access the cave, check whether the area is available in your game and progress the relevant Archon Quest if needed; the prerequisite is not settled by the available sources.
Achievement reward and optional cave loot
GameWith reports that the achievement awards 5 Primogems. It also reports four chests in the cave worth 80 Primogems total, plus a Dendroculus near the fourth chest. Those chest and reward figures are guide-reported, not confirmed here by official documentation. Collecting the chests and fighting optional enemies are not necessary to reach the exit and hoe interaction.
