Open that Sim’s makeover option. An older accepted answer on EA Forums calls it “Extreme Makeover”; the answer does not verify that this label or menu path is still current.

In the makeover screen, look for the surname field and edit it. The older forum answer says the makeover menu allows name and appearance changes, but EA’s current Sims FreePlay help hub does not document this procedure.

Review the displayed edits and cost in your game before confirming. The forum discussion mentions that prices changed but does not establish a current amount; use the price shown in your game.
What happens to the rest of the household?
The accepted forum answer says changing an adult Sim’s surname changes the surnames of all Sims living in the same household. Later replies in that discussion describe cases where a spouse or children did not update consistently, so treat a household-wide change as a possibility rather than a guarantee.

If another household member keeps a different surname, check that Sim’s own makeover options instead of assuming the game applies one surname to everyone.

The procedure comes from an older community-forum answer, not a current official EA how-to. EA’s official game page identifies the mobile game and its updates but does not explain surname editing. The current menu label, exact steps, cost, and household-wide behavior are therefore not established by official documentation.
