Taken together, those plot points make Downes the implied source of Arthur’s illness. The game does not present a separate, medically verified account of exactly when Arthur became infected or prove that transmission happened during that particular encounter.
Did Thomas Downes give Arthur TB?
That is the story’s clear implication: Downes is the sick person Arthur encounters, and the game later connects Downes’ death from TB with Arthur’s diagnosis. It is best described as the narrative explanation, rather than a confirmed biological account of transmission.

How TB spreads in real life
The CDC explains that TB germs can spread through the air when someone with active TB disease of the lungs or throat coughs, speaks, or sings. Another person may become infected by breathing in those germs. Germs can remain in the air for several hours depending on the environment, and spread is more likely indoors or where air circulation is poor. TB infection and active TB disease are not identical. CDC guidance describes active disease as occurring when the immune system cannot keep TB germs from multiplying. The game’s diagnosis is part of Arthur’s story; it should not be treated as a detailed account of real-world diagnosis or disease progression.
