Recruit a survivor who already has the Immortal trait. The available references do not document a guaranteed mission, unlock, or normal gameplay action that adds Immortal to a survivor you choose.
How to get a survivor with Immortal
Check the traits of survivors you encounter and recruit one who already has Immortal. The State of Decay 2 Wiki’s Traits reference lists Immortal as a survivor trait, but does not establish a guaranteed spawn method, dedicated mission, or in-game item for selecting it.

You can consider ordinary recruitment and survivor-search strategies, but treat them as attempts rather than reliable ways to produce Immortal. No appearance rate is established by the available references, so there is no supported probability or number of recruits to promise.
What the Immortal trait does
The Traits reference lists these personal effects for Immortal:

These values are listed by a community-maintained reference; they are not presented here as independently tested figures. The official State of Decay 2 Player Guide provides general game and survivor-management context, rather than establishing a way to obtain this particular trait.
Immortal and its Hero Bonus are different
The Traits reference associates Immortal with the Hero Bonus Taking Risks, listed as +5 maximum health. The trait’s personal effects and the community benefit are separate: having Immortal does not by itself mean the bonus is already active.

According to the Wiki’s Hero Bonuses and Community Standings references, a survivor’s Hero Bonus is determined when that survivor is generated and does not change. It is hidden while the survivor is a recruit, becomes visible at Citizen standing, and activates when the survivor reaches Hero standing.

The references reviewed do not establish a normal-play method for adding Immortal to a chosen survivor. They list the trait and its effects, but do not document it as a quest reward or selectable upgrade. Community editors or mods are not normal in-game methods and should not be confused with recruiting a survivor who already has the trait.
