In Terraria’s Calamity Mod, summon Anahita in the Ocean first; the Leviathan joins the fight when Anahita reaches 40% health. You can start the encounter by defeating an Ocean enemy or by using a Naiad’s Warhorn in the Ocean. The fight is intended for after Plantera, but the Calamity Mod wiki says it can be started earlier.
How to start the encounter
The encounter begins with Anahita. The official Calamity Mod wiki’s Leviathan and Anahita entry lists two ways to summon her, both associated with the Ocean:

Crafting the Naiad’s Warhorn
If you choose the item route, craft the Warhorn using the recipe listed on the official Naiad’s Warhorn page:
- 10 Driftwood
- 15 Abyss Gravel
- 10 Lumenyl
- A Mythril or Orichalcum Anvil
Take the finished Warhorn to the Ocean and use it there to summon Anahita.

When the Leviathan appears
Keep fighting Anahita after she is summoned. The Leviathan appears when Anahita reaches 40% health, according to the Calamity Mod wiki encounter entry.

The encounter is intended to be fought after Plantera, although the wiki says it can be summoned at any point. Its mechanics have changed between mod versions: the history page records a rework in version 1.4.5.001 that makes the Leviathan appear at 40% of Anahita’s health on all difficulties, and also lists a later 1.4.5.002 entry. If your game behaves differently, check the wiki for the version of Calamity Mod you have installed.
