It starts the event without requiring the conditions for a natural spawn. If you do not have a Bloody Tear, you must wait for an eligible night and hope the event triggers at dusk.
How to summon a Blood Moon with a Bloody Tear
- Have a Bloody Tear in your inventory.
- Wait until nighttime, then use the item.
- Look for the message “The Blood Moon is rising…” to confirm the event has started.
A Bloody Tear cannot start or change a Blood Moon during the day or while one is already active. In either case, it is not consumed. Natural-spawn requirements—including player health and moon phase—do not apply when you use the item. You can even use a Bloody Tear during a New Moon; doing so advances the phase to a Waxing Crescent. Official Terraria Wiki: Bloody Tear

How Blood Moons spawn naturally
The game checks for a natural Blood Moon at dusk. At least one player in the world must have more than 120 health, the moon phase must not be a New Moon, and the Eye of Cthulhu must not be due to spawn that night. If all three conditions are met, the event is still not guaranteed:
- Regular worlds: a 1/9 chance (11.11%) on each eligible night.
- Celebration Mk 10 or Zenith worlds: a 1/6 chance (16.67%) on each eligible night.
Each eligible night is a separate chance; having had a Blood Moon recently neither guarantees nor rules out another one. Official Terraria Wiki: Blood Moon

Where to get a Bloody Tear
Bloody Tears drop from certain enemies. Listed chances vary by enemy:

| Dreadnautilus | 50%; 100% in Expert Mode |
Most listed enemies are associated with the Blood Moon, but there are exceptions: The Bride and The Groom can drop Bloody Tears outside the event, and the Dreadnautilus can drop one at any time. Version and platform note
Blood Moons and Bloody Tears are documented for Desktop, Console, and Mobile versions. The item was introduced on Desktop in version 1.4.0.1 and later appeared on Console and Mobile. Mechanics on legacy console and 3DS releases may differ, so the instructions here should not be assumed to apply identically to those older versions.
