In Tomodachi Life for Nintendo 3DS, you can set each Mii’s relationship to your own Mii when creating or editing that character. Nintendo’s instructions do not document a direct way to declare two other island Miis siblings or otherwise related.
Set a Mii’s relationship to your own Mii
- Open Town Hall, or go to the room of a Mii you created. Nintendo says you can edit your Miis in either place.
- Open the Mii’s creation or editing flow and set its relationship to your own Mii. Nintendo’s Tomodachi Life quick guide specifically reminds players: “Don’t forget to set the relationship to your own Mii for each Mii!”
This setting is player-relative: it tells the game how that character relates to your own Mii. The guide does not provide a setting for choosing another island Mii as the relative.

Can you make two other Miis siblings?
Nintendo’s official 3DS instructions reviewed here do not give a verified method for directly assigning a family relationship between two other island Miis. That is a limit of the documented instructions, not proof that the game can never recognize relatives through other behavior. Avoid treating instructions for a different edition or unverified community advice as official 3DS steps.

Nintendo’s 3DS manual explains how to view relationship information from inside an apartment using the indicated directional control. That lets you check what the game displays; it does not assign or change a relationship.

Check which Tomodachi Life edition you’re playing
This answer applies to Tomodachi Life on Nintendo 3DS. Nintendo also documents Tomodachi Life: Living the Dream for Switch and Switch 2, with separate Mii creation and dating-preference options.
