Outlast Review: Is the 2013 Survival-Horror Game Worth Playing?

Outlast trades combat for hiding and running in a frightening asylum. Here’s what works, what may wear thin, and what to know before choosing a version.

Outlast is a first-person survival-horror game built around being hunted, not fighting back. Its strongest case is a frightening asylum setting and tense stealth; its main caveats are repetitive chase sequences and content that includes intense violence, gore, sexual content, nudity, and strong language. Whether it is worth playing depends on how much you enjoy helpless, hide-and-run horror.

What is Outlast?

Released on September 4, 2013, Outlast was developed and published by Red Barrels. You play investigative journalist Miles Upshur, who enters Mount Massive Asylum after receiving an anonymous tip. The asylum has ostensibly reopened under the Murkoff Corporation’s research and charity branch; Miles’s investigation turns into a fight to uncover what is happening and escape. Red Barrels’ game page describes it as survival horror.

It is a single-player game. Rather than giving Miles a conventional way to defeat enemies, Steam’s description emphasizes stealth, hiding, running, and parkour-inspired movement. That vulnerability is the core of the experience: success comes from avoiding threats and finding a route onward, not clearing rooms through combat.

Is Outlast scary?

Its horror comes from a combination of an oppressive asylum, sound, pursuit, and the knowledge that Miles cannot simply fight his way out. Critics cited by Metacritic praised the atmosphere, sound design, and tension created by vulnerability. That makes Outlast a natural fit for players who want to feel hunted and prefer stealth-based fear to combat-driven action.

The scare style is not universally effective. Some critics found the cat-and-mouse sequences repetitive, while another criticized reliance on jump scares and visuals that felt underwhelming. In its reproduced review, Gameplanet wrote: “Outlast gets the visuals and sound right, has some great mechanics, and will provoke feelings of dread and horror in even the most jaded gamer. It’s cat-and-mouse sequences are a repetitive dirge, but elsewhere this is a strong first offering.” The line captures the trade-off: the atmosphere can work well, even if the repeated pursuit pattern or sudden scares do not suit every player.

What do reviewers think of Outlast?

Metacritic listed Outlast at 80/100 from 59 critic reviews when accessed in 2026. That aggregate is a snapshot of critic scores, not a definitive measure of whether the game will suit an individual player. The reviewed excerpts broadly support its reputation for atmosphere and sound while also recording reservations about repetition, jump scares, and visuals.

Steam’s English user-review listing showed “Overwhelmingly Positive,” with 96% positive from 27,184 reviews when accessed in 2026; its recent English reviews showed 97% positive from 1,377 reviews at that time. Who should play it—and who might not?

Outlast is most likely to work for players who enjoy stealth, evasion, and sustained tension more than combat or a broad range of encounters. If being chased repeatedly sounds like a drawback rather than a source of suspense, the critical reservations about repetition are worth taking seriously.

Content is a major consideration, not a mild-horror footnote. Steam’s developer-supplied warning lists intense violence, blood and gore, sexual content, nudity, and strong language. The official US PlayStation page lists the same categories. If any of these subjects are a concern, check the current content information on the store for the version and region you are considering.

Where can you play Outlast?

The game is listed for PC on Steam and for PS4 on the US PlayStation storefront. That PlayStation listing says the PS4 version is playable on PS5, though some PS4 features may be absent. Store availability, compatibility details, and prices can vary by region and change over time, so check the listing for your platform before buying. Steam also provides its PC system requirements on the game’s store page.

Does Outlast have a physical edition?

Yes, but documented physical releases are collections or historical limited editions, not evidence that new copies are currently in stock. Warner Bros. Interactive Entertainment announced Outlast: Trinity on March 6, 2017, as a physical bundle for PS4, Xbox One, and PC containing Outlast, Outlast 2, and the Whistleblower downloadable content. Warner Bros.’ announcement documents the bundle.

For Nintendo Switch, Nintendo Life reported that Limited Run Games announced physical editions of Outlast and Outlast 2, with preorders opening October 26, 2018. Nintendo Everything reported standard physical copies and a Murkoff Briefcase collector edition containing both games, a soundtrack CD, and a graphic novel. These were historical releases; current availability is not established.

If searching for a physical copy, “Outlast Trinity physical game” is a useful search phrase, but it refers to the collection rather than the digital standalone game. Before purchasing any listing, verify its platform, included games and content, region, condition, seller, and current availability.

Verdict: Is Outlast worth playing?

Outlast is worth considering if you want a focused survival-horror game in which hiding and escaping create the danger. The critic reception supports its atmosphere and sound, while the chase-loop criticism points to the clearest reason it may not click. Check the content warnings first, and choose a digital or physical edition based on the platform and included content you actually want.
