There is no single best painkiller for everyone. In the U.S., common over-the-counter options include acetaminophen and the NSAIDs ibuprofen, naproxen, and aspirin. The useful choice depends on whether inflammation is involved, the product label, your health conditions, and other medicines you take. This is a general comparison, not individualized dosing advice.
What counts as a painkiller?
“Painkiller” is a broad term for medicines that relieve pain. Over-the-counter (OTC) products commonly use acetaminophen or a nonsteroidal anti-inflammatory drug (NSAID). Aspirin, ibuprofen, and naproxen are NSAIDs. Prescription opioids are a separate category, not another OTC alternative.

Choosing between acetaminophen and an NSAID
When pain or fever is the main concern
Acetaminophen may be considered for common aches or fever when inflammation is not the issue. It does not reduce inflammation, so it is not interchangeable with an NSAID for every symptom or situation.
When swelling or inflammation is involved
NSAIDs such as ibuprofen, naproxen, and aspirin can relieve pain and reduce swelling. That benefit does not make them suitable for everyone: stomach risks, health conditions, and interactions with other medicines matter.

When the choice is unclear
Compare the active ingredient, strength, formulation, package directions, and warnings rather than relying on a brand name. Ask a pharmacist or clinician when you are unsure which ingredient fits your situation.
Safety checks before taking an OTC pain reliever
- Read the active ingredients on every medicine. Cold, flu, and combination products may contain the same pain-relieving ingredient. Taking multiple acetaminophen-containing products can result in too much acetaminophen.
- Follow the package directions. Do not exceed the label or a clinician’s instructions. The FDA advises consumers to follow directions when using common OTC pain relievers and fever reducers; taking more than recommended can cause serious injury. - Check health conditions and other medicines. MedlinePlus highlights the relevance of heart, blood pressure, kidney, liver, or digestive problems and medicines such as blood thinners. Ask a pharmacist or clinician whether a product is appropriate for you.
- Do not combine NSAIDs without checking. In particular, NHS guidance says not to take naproxen at the same time as ibuprofen or aspirin. Follow local labeling and professional advice.
- For children, use child-specific directions. Do not give a child an adult medicine; follow the child-specific product instructions or ask a healthcare provider.
When to ask a healthcare professional
Contact a pharmacist or clinician if dosing is unclear, a long-term condition or another medicine could affect your choice, symptoms are severe, or you find yourself needing pain medicine repeatedly. Individual guidance matters because products, formulations, labels, and health histories differ.

How prescription opioids differ
Opioids are prescription medicines that may be used for moderate to severe pain, including after major injury or surgery. They require provider supervision because of serious side effects and addiction risk. They are not equivalent to the OTC ingredients compared above.
