What does Confusion do in Nioh 3?
The Nioh 3 Wiki’s combat guide describes Confusion as a status interaction: build two different elemental statuses on one enemy, and Confusion is triggered. It also describes the elements’ individual effects as follows:

- Check the target’s resistances. Choose two different elements the boss is not strongly resistant to, using the information available in the game and the guide’s resistance advice.
- Prepare two distinct ways to build elemental status. The third-party guide identifies weapon or talisman use and Ninjutsu as possible application routes. Team NINJA’s official combat overview confirms that Ninjutsu is part of the Ninja toolkit, but neither source establishes one universally best application method.
- Wait for a safe opening. Try to apply both elements when the boss is recovering, staggered, or otherwise unlikely to punish you before the second status builds. Avoid trading hits just to finish the setup.
- Use the opening rather than forcing a fixed combo. Once Confusion is active, attack while watching your own Ki and the boss’s recovery. Stop or reposition when the boss is about to retaliate.
The sequence is a decision process, not a guaranteed rotation: resistance, available status sources, and the length of the opening vary by encounter. Team NINJA says Ki Pulse recovers some Ki after attacks and that changing stance affects moves, damage, and Ki consumption; those systems can help you manage the follow-up, but they do not establish a specific Confusion combo.
Which elements should you use?
Choose based on both resistance and the kind of opening you can exploit. Fire adds ongoing damage, Water reduces defense, and Lightning slows actions and Ki recovery, according to the third-party guide. The practical choice is the pair you can apply reliably without exposing yourself to a punish—not a fixed element ranking.

Application method is also encounter-dependent. A weapon or talisman may fit a close-range opening; Ninjutsu offers another route if it is part of your setup. The cited sources do not compare their buildup speed or establish a best method for every boss.
How many Confusion levels are there, and how much extra damage does it cause?
The sources cited here do not verify the number of Confusion levels, their triggers, their damage multipliers, status-buildup thresholds, or duration by level. Do not treat community percentages or Nioh 2 mechanics as confirmed Nioh 3 values.

Team NINJA’s Nioh 3 updates page surfaced a patch-history reference to a Confusion-duration set bonus and an adjustment involving renewal of Confusion and duration bonuses. That indicates duration-related mechanics exist, but the available update information does not establish tier math or damage values.

No single boss location or attack window is established as universally best. Look for a phase or recovery that gives you enough time to apply both statuses, then take advantage of a Ki-depleted or staggered state if the encounter permits it.

A PlayStation.Blog hands-on report from November 21, 2025 describes a fight against Nuribotoke in a Crucible: the writer used Samurai attacks to reduce the boss’s Ki, then switched to Ninja for damage during openings, with Guardian Spirit abilities and Soul Core summons also contributing. The report does not say Confusion was applied, so it is an example of exploiting openings—not evidence of a Confusion-specific route or best location.
How Samurai and Ninja can support the setup
Team NINJA describes Samurai as emphasizing powerful techniques and precisely timed Deflect, while Ninja emphasizes agile movement, aerial actions, and Ninjutsu. Treat these as complementary tools for creating openings and applying statuses, not proof that one style or build is optimal for Confusion. A boss’s attack pattern and your available elemental sources determine when each tool is useful.
