Defeating the Delirium encounter that follows lets you return to Tujen for the quest reward.
How to start Dark Mists
Speak with Tujen in Kingsmarch and accept his task to investigate the mist. Then ask Makoru to sail to Kedge Bay. Explore the island and enter Journey’s End.
How to get the Verisium Spikes
- Continue exploring Journey’s End until the quest prompts you to summon Tujen and search for Captain Hartlin.
- Defeat Captain Hartlin and collect the Verisium Ingots he leaves behind. They are needed for the next step.
- Return to Kingsmarch and speak with Dannig, bringing him the ingots. He provides Verisium Spikes for use at Journey’s End.
How to free Freya Hartlin
Return to Journey’s End and find Freya Hartlin, who is bound by Karui Totems. Use the Verisium Spikes on the totems around her; the quest guide lists five. Freya’s release starts a Delirium encounter.
How to finish the Delirium encounter
Defeat Omniphobia, Fear Manifest. Once the encounter is complete, return to Tujen in Kingsmarch to claim the reward.

Dark Mists quest rewards
As of Path of Exile 2 update 0.4.0, the quest grants a Mist-shrouded Tome, an Uncut Skill Gem, and Gold. Using the tome grants two Weapon Set Passive Skill Points. Grinding Gear Games announced the change in its 0.4.0 patch notes: “The Blind Beast no longer drops a Bile-soaked Tome. Instead, the Dark Mists quest now grants a Mist-shrouded Tome (which grants two Weapon Set Passive Skill Points) in addition to its existing rewards (Uncut Skill Gem and Gold).”
The quest database lists the Uncut Skill Gem as level 55. Reward details here reflect the 0.4.0 change; later updates could alter Act 4 or the quest, so check the current in-game reward if playing on a newer patch.
