The effect is not a recurring five-minute cooldown in the updated description. Blizzard says it is disabled after use until you return to town, replacing the prior wording that allowed it to activate once every 300 seconds. The return-to-town reset is the current behavior stated in those notes.

What does Mythic mean for this item?
Blizzard’s 3.1 PTR overview describes Mythic as a modifiable Item Quality for Season 14 and says any Unique may become Mythic, either by dropping that way or through an upgrade using the Horadric Cube. How do you get Shroud of False Death?
The official sources cited here do not establish a complete current, item-specific acquisition route, drop location, or drop rate. The PTR overview discusses Mythic upgrades generally, but does not confirm a live method or cost for obtaining this particular chest armor.
