It adds one Resurrective Power node, and the documented reward is the same from either recipient. You do not need to save it for an ending.
What happens when you give the Sakura Droplet?
Presenting the Sakura Droplet to Kuro or the Divine Child of Rejuvenation increases your Resurrective Power by one node. The item’s reference lists both characters as recipients and describes the same benefit for each. Sekiro’s item reference also records the Divine Child saying, “It appears the Sakura Droplet’s power now resides within you.”
There is no documented recipient-specific gameplay advantage. Choose whichever character is available and convenient.
How does the extra resurrection node work?
The node adds capacity to the existing Resurrective Power system; it is not an unrestricted extra life. Resurrection spends Resurrective Power, as described in FromSoftware’s game manual. Activision’s gameplay guide explains that resting at a Sculptor’s Idol restores one charge and that additional charges can return through combat.

Does giving it affect an ending?
No ending effect is established for giving the Sakura Droplet. The item-specific reference describes an additional Resurrective Power node, not an ending choice. A claim that giving it unlocks Immortal Severance or an alternate route appears in a 2024 SplicedOnline guide, but conflicts with the documented item effect; do not use that claim to decide whether to give it.
Is the Sakura Droplet the same as the Dragon’s Blood Droplet?
No. The Sakura Droplet increases Resurrective Power when presented to one of its documented recipients. The similarly named Dragon’s Blood Droplet is associated with restoring characters affected by Dragonrot. The game’s manual and Activision’s guide discuss the broader resurrection and Dragonrot mechanics; the item reference distinguishes the Sakura Droplet’s effect.
