Spoilers for Act Three and the final confrontation. To free Orpheus in Baldur’s Gate 3, obtain the Orphic Hammer, reject the Emperor’s plan to assimilate him, and use the Hammer to destroy both crystals binding him in the Astral Prism. You can accept Raphael’s bargain for the Hammer or refuse and steal it from the House of Hope. Once Orpheus is free, someone must become a mind flayer to wield the Netherstones.
How to get the Orphic Hammer
The quest to free Orpheus takes place in Act Three. Speaking with Kith’rak Voss or Raphael at Sharess’ Caress can set you on the path: Voss wants to break Orpheus’s chains, while Raphael tells you he has the Orphic Hammer. The Hammer is what you need to break those chains. BG3 Wiki: Free Orpheus
You have two main ways to obtain it. The choice determines whether you are bound by Raphael’s contract or have to face the dangers of his House of Hope. BG3 Wiki: Orpheus

How to free Orpheus in the Astral Prism
- Have the Hammer ready. Bring the Orphic Hammer to the final confrontation. When the Emperor takes the party into the Astral Prism, make sure it is available to the active party. BG3 Wiki: Confront the Elder Brain
- Reject the Emperor’s plan. The Emperor wants to assimilate Orpheus and use his power. Rejecting that proposal makes the Emperor leave to rejoin the Netherbrain. BG3 Wiki: Confront the Elder Brain
- Break both binding crystals. Equip the Orphic Hammer and strike each of the two crystals holding Orpheus. Both must be destroyed to free him. BG3 Wiki: Confront the Elder Brain
Who becomes a mind flayer?
Freeing Orpheus does not remove the need for a mind flayer: Orpheus explains that one must wield the Netherstones. Your player character can take that role, or Karlach can if she is available. If neither does, Orpheus transforms. BG3 Wiki: Confront the Elder Brain

What if you refused Raphael and do not have the Hammer?
Do not assume the Hammer will always be recoverable at the end. If Raphael is still alive, he may make a last offer in the Astral Prism. If he is dead and the active party does not have the Hammer on entering the Prism, the documented outcome is a game over.
