No—not as an established rule of Fallout. The franchise shows both lucid, non-feral ghouls and feral ghouls, but Bethesda’s official overview does not say that every ghoul will eventually lose their mind. Neither a universal cause nor a timetable for feralization is established in the sources cited here.
What does Fallout establish about ghouls?
Bethesda describes ghouls as humans changed by radiation and chems, and says the games depict them across a range from wise survivors to terrifying feral monsters. That confirms both conditions exist; it does not establish that one inevitably leads to the other. Bethesda’s ghoul overview
So the careful answer to “Do all ghouls eventually turn feral?” is that the available official material does not support a universal yes. A character may believe feralization is inevitable, but that belief should be attributed to that character rather than treated as a franchise-wide rule.
Why do some ghouls become feral while others remain lucid?
The official overview does not identify a specific mechanism that turns a non-feral ghoul feral, or explain why some retain their minds while others do not. Radiation and chems are associated with becoming a ghoul; that is not the same as establishing what causes a ghoul to later become feral.

Fans have proposed explanations including radiation, time, isolation, and individual differences. These remain interpretations rather than confirmed rules. A Fallout community discussion reflects the debate, not an authoritative answer.
Is there a known timetable for feralization?
No supported timetable or rate is established in the official material cited here. It does not say that a ghoul becomes feral after a certain number of years, at a particular age, or after a measurable amount of radiation exposure. Any exact deadline or guarantee should therefore be treated as speculation unless a specific story establishes it.

Does the answer apply to both the games and the TV series?
Bethesda describes the Fallout games and television series as sharing the Fallout universe. That supports treating the series as part of the same broad setting, but it does not make every proposed explanation for feralization settled across every installment. For a particular claim, look at what the relevant game, character, or episode actually shows or says. The existence of feral ghouls in one story does not by itself prove that every lucid ghoul will eventually become one.
