Fallout 76: Ghoul Within launched on March 18, 2025. Its “Leap of Faith” quest gives eligible level-50 characters the choice to become ghouls. The launch-day “now live” announcement is historical; Bethesda later changed some ghoul mechanics in its June 2, 2026 update.
What the Ghoul Within update added
Bethesda announced the update on March 18, 2025, saying it went live that day. In the announcement, Associate Content Manager Gabriel Santiago wrote: “Fallout 76 now gives players the choice to transform into a ghoul.” Bethesda’s launch announcement introduced ghoul transformation as the update’s central feature.

The update also added 30 ghoul-specific Perk cards—28 regular cards and two legendary perks—along with systems built around Glow and Feral. Ghouls do not manage hunger and thirst in the same way as human characters. These are distinct mechanics, not a simple upgrade to the existing human playstyle. Bethesda’s March 18, 2025 release notes detail the launch features.
How to become a ghoul
Complete “Leap of Faith”
The quest “Leap of Faith” takes players to a new landmark in the Savage Divide and leads to the transformation choice. Bethesda’s launch announcement identifies level 50 as the eligibility point for choosing to become a ghoul. Consult the in-game quest and eligibility prompts for your character’s current options.

- New Main Quest: Uncover the secrets of West Virginia by playing through an all-new main quest, starting from the moment you leave Vault 76.
- Human NPCs: Befriend or betray new neighbors who have come to rebuild, and experience Appalachia through the eyes of its residents.
- New Creatures and Gear: Take down newly mutated creatures finding their way to the region and earn the highest tier of weapons and armor.
- Choices and Reputation: Alter the fates of those you meet with dialogue trees. Decisions affect your standing with each faction through an all-new reputation system.
Human or ghoul: what changes?

These paths suit different preferences. A ghoul offers its own perks and survival systems, with possible faction-access consequences; a human character avoids the ghoul-specific mechanics. Bethesda’s notes establish tradeoffs, not a universally better build.

Bethesda’s June 2, 2026 Infestations release notes supersede the launch-era descriptions for the specific radiation and Glow behavior they address. Under those notes, player ghouls convert 35% of radiation exposure damage remaining after resistance into health; once at full health, that converted amount goes to Glow. Bethesda also changed default player-ghoul radiation resistance and the Rad Specialist perk. The 35% figure describes this conversion rule, not a general percentage of all incoming radiation damage before resistance.
