Deckard Cain dies during Diablo III. His later appearance in Diablo IV does not undo that event: Blizzard describes him as a “lost soul” who returns to Sanctuary to guide the seasonal story of Hell’s Legacy.
Where Cain fits in the Diablo timeline
Cain is a recurring Horadrim scholar. Blizzard places Diablo Immortal between Diablo II: Lord of Destruction and Diablo III, and says Diablo III takes place twenty years after Diablo II. Cain is among the familiar characters in Immortal; by the time of Diablo III, he is focused on the End of Days prophecy. (Blizzard’s timeline and story overview.)

- Cain survives the events of Diablo II and remains part of Sanctuary’s story during the period covered by Diablo Immortal.
- He is present at the start of Diablo III, researching the End of Days prophecy.
- Cain dies during Diablo III. Blizzard’s cited overview establishes the broader setting, but does not recount the death scene or its dialogue.
- In Diablo IV seasonal material, Cain returns as a lost soul rather than through a stated reversal of his death.
Why Deckard Cain appears in Diablo IV
Blizzard’s overview of Diablo IV’s Season of Hell’s Legacy says, “Amid the dead rising from Pandemonium, the lost soul of the legendary Horadrim Deckard Cain returns with dire news.” The season’s story draws on lost souls and memories from Sanctuary’s past, so Cain’s role is framed as a soul returning to guide the seasonal questline—not as proof that he survived Diablo III or was resurrected. (Blizzard’s Season of Hell’s Legacy overview.)
A separate Blizzard panel recap says Cain’s “lost soul walks in Sanctuary once again – although a bit confused by the journey.” That wording supports his return as a soul, but does not explain where he went after dying or the mechanics of his journey. (Blizzard’s Diablo IV panel recap.)

Did Cain’s death get reversed?
No reversal is established by Blizzard’s seasonal descriptions. They call Cain a lost soul and place him in a story about souls from Sanctuary’s past. The most precise reading is that Cain dies in Diablo III and later appears in Diablo IV as a lost soul; the sources do not claim his death never happened.
