They do different jobs, so the choice depends on whether you want OptiFine’s features, Forge’s mod ecosystem, or a compatible combination of the two.
OptiFine and Forge do different jobs

That distinction matters: Forge is not an alternative graphics optimizer, and OptiFine is not a general-purpose mod loader. The projects can be used together in some setups, but compatibility depends on the specific Minecraft, Forge, and OptiFine releases.
Which one should you choose?
Choose OptiFine for its features
If your goal is OptiFine’s performance options or visual features, start with the OptiFine release for your Minecraft version. Its FAQ says that Ultra is its most popular edition and has the most optimizations and features; Standard is described as more compatible with other mods; and Light lacks advanced features and is incompatible with ModLoader and Forge. These are OptiFine’s own descriptions, so check the exact release and your other mods before choosing an edition. See the OptiFine FAQ.
The same FAQ says OptiFine “generally increases the FPS (200%+ is common) and in the worst case it should be the same as vanilla Minecraft.” That is an OptiFine claim, not an independently verified benchmark or a promise about performance on your computer.

Choose Forge to run Forge mods
If the mods you want are made for Forge, install Forge for the intended Minecraft version and launch the game using that Forge installation. Forge’s documentation explains its mod-file setup in The Mod Files.
Use both only when the releases match
You may be able to use OptiFine with Forge, but the combination is not guaranteed to work for every release. Check the official OptiFine Downloads page for the relevant Minecraft release; it lists Forge version information for some entries and “N/A” for others. Treat that release-specific listing as more useful than a general assumption that OptiFine works with Forge.

How to install OptiFine with Forge
OptiDocs’ Install With Forge guide gives this general procedure, subject to version compatibility:
- Install Forge for the Minecraft version you intend to play.
- Download the OptiFine release for that Minecraft version and follow OptiFine’s download instructions.
- Place the OptiFine JAR file in the Minecraft
modsfolder used by the Forge installation. - Launch Minecraft using the Forge profile or installation.
Before launching, compare the exact Minecraft and Forge versions with the OptiFine entry on the official downloads page. A matching Minecraft version alone does not establish that a particular OptiFine and Forge combination is compatible.

Where to get help if the combination fails
Forge’s FAQ says Forge does not support OptiFine issues because OptiFine is closed source and difficult to debug without its source. For a problem specific to OptiFine, use OptiFine’s own support routes. If the issue occurs only after adding other mods, check the OptiDocs troubleshooting guide, which notes that other installed mods can conflict.

- Confirm that Minecraft, Forge, OptiFine, and any other mods target compatible versions.
- Test whether the issue remains with only Forge and OptiFine installed; other mods may be responsible for a conflict.
- For an OptiFine-specific problem, contact OptiFine rather than expecting Forge to debug the closed-source mod.
