Grindylow Tooth is enemy loot from Grindylows. For the Age of Excuse quest, start hunting west of the Archspire Fast Travel Gate; the tooth is not a guaranteed drop, so you may need to defeat more than one.
Where to hunt Grindylows for the quest
The Age of Excuse walkthrough points to an area west of the Archspire Fast Travel Gate. It places the quest near Berach, under a bridge northeast of the gate, where Drest gives you the Disgusting Health Potion recipe. Use the west-of-gate location as your starting point, then search nearby for Grindylows.
If you do not find enough there, the Tainted Grail Wiki’s Grindylow entry lists encounters across the Horns of the South and Cuanacht regions, with encounters in the Horns of the South especially near water.
How to get the tooth
Defeat Grindylows and check their loot for a Grindylow Tooth. The item is listed as a non-craftable reagent used in Alchemy and Handcrafting; the wiki also lists identifying items as a source. The quest walkthrough says Grindylows do not always drop the tooth. It gives no numerical drop rate, so there is no supported kill count or special weapon requirement to rely on.

One Steam discussion user reported killing 20 to 30 Grindylows without a drop. That is an individual report, not evidence of a typical drop rate or proof that a particular weapon or method is required. A community description calls Grindylows “the little dudes with the hammers”; treat that as a rough visual clue, not an authoritative creature description.
How many teeth the Age of Excuse quest needs
The Disgusting Health Potion requires one Grindylow Tooth, two units of alcohol, and five Sage. The tooth is only one part of the ingredient list, so collect the other materials as well before returning to the quest.
