Pestilence Outbreaks have no fixed spawn point in No Rest for the Wicked. Any map area can become infested, so the useful question is which area is eligible right now. To start an outbreak yourself, travel to a fire in the area you want and burn material there, as long as that area’s cooldown has expired. A community guide on the game’s forum describes this process; its details are below, along with what the guide and later player reports say about the map.
Why there is no single outbreak location
The community guide by Silas-Inservio-Pax, posted on the No Rest for the Wicked forum on September 15, 2025 and updated February 4, 2026, states that every map area can become infested. An outbreak is therefore a state of a specific area at a specific time, not a landmark you can memorize. If you are checking where Pestilence is active, check the area you are in or planning to visit.
How to start a Pestilence Outbreak in a chosen area
The guide says any material can be burned at a fire to start an outbreak in the area containing that fire. The steps are:
- Open the map and look at the area you want. Note the “Area Cleansed” timer shown at the upper left of the map, in the form “Area Cleansed xx hours xx minutes”. If that timer is running, the area is in cooldown and cannot be triggered yet.
- Travel to a fire or bonfire inside that area.
- Use the burn option at the fire and burn material. The guide notes that burning more valuable material is what raises the outbreak tier, covered below.
- Follow the outbreak markers on the map and defeat enough of the area’s enemies. The guide says you need to defeat a percentage of the inhabitants, not every enemy; it does not give a fixed percentage.
- Once enough enemies are cleared, the area’s outbreak boss can appear.
The guide says manually triggering an outbreak follows the same 24-hour wait as a natural occurrence. Because the guide was last updated in February 2026, check that the timer still behaves this way if a later patch has changed the system.

Three ways to reach the outbreak boss compared
Players have three practical routes. The guide describes each as follows:

The trade-off is time versus cost. Waiting costs nothing but time. Burning material gets you an outbreak on your schedule but consumes that material and may raise difficulty. The Torn Effigy skips straight to the boss, so you will not have to clear the surrounding enemies first, but the guide does not describe its effect on tier.

What outbreak tiers change
According to the same guide, burning materials raises the outbreak tier. Higher tiers bring negative modifiers, and difficulty also affects the flat boss rewards. The guide makes a specific distinction about rewards: higher tiers do not increase enemy loot drops or chest rewards and spawns. It does report an increased chance for Legendary and Unique drops. If your goal is more loot from chests or enemies, a higher tier is not the lever the guide describes. Players who want the Legendary and Unique drop boost should weigh it against the added modifiers.
When the outbreak does not show on your map
Map problems have been reported, but these are player observations rather than official confirmations.

- Cooldown not finished. If an area’s “Area Cleansed” timer is still running, that area is not eligible for another outbreak. This is the first thing to check.
- Missing progress bars or markers. A player report in a forum bug thread dated September 2, 2026 describes absent progress bars and faulty or missing boss markers at Nameless Pass: Ashen Road Whisper and Lowland Meadows: Windmill Whisper. The same thread describes boss encounters around The Shallows: Saltborn Tower and Orban Glades: Forgotten Bridge. These are individual reports, not a developer-confirmed bug notice.
- Purple circles. In replies within that thread, forum participants said a purple circle marks a cave containing Pestilence enemies. Those enemies may need to be cleared to reduce an area’s infestation, and it is easy to overlook them while focused on the boss. One participant said a small skull marker should indicate the boss. These are community explanations, not official definitions.
- Story-trigger anecdote. A May 2025 player bug report said that teleporting away after the story introduction, instead of walking back through the cave, seemed to stop the event prompt from appearing in that player’s game. It was a single report, not a confirmed fix, so do not rely on it for a current save.
Using a third-party map
A Reddit announcement describes an interactive community map called NRFTW Tools, which includes a Pestilence Outbreaks toggle and a campaign progression toggle. It is useful for locating areas quickly. Because it is a third-party tool and outbreak states change over time, treat it as a navigation aid and confirm the area’s timer on your own in-game map before traveling.
Keep in mind that the guide’s cooldown and tier information is community-sourced and dated; the in-game map and timers are the authoritative check for your own save.

Do not treat a map marker as a guarantee: a marker that appears wrong may be a bug, so verify with the area’s timer and the enemies you can see.
If an outbreak still does not appear after the cooldown has cleared, the reported bugs above are the most likely cause.
Use the same caution with any boss marker you see.
