Chapter 18, Belly of the Beast, has six secrets: four Gold pickups, a Ruby, and a hidden route with Gold and a Life Sigil. Follow them in order, and finish the optional stomach encounter before crossing any point of no return. The chapter also has other collectibles and challenges; those are listed separately so they are not confused with the six secrets.
All six secrets in chronological order
Use the AutoMap to check nearby unexplored spaces and secret areas. The route below follows the chapter’s progression; watch for blue and green symbols that signal Shield Saw traversal.

1. First organ area — 50 Gold
After clearing the first encounter, interact with the organ to advance the story. Turn left to find the gold chest.
2. Near the objective door — 6 Gold
From the first chest, turn around and follow the left wall toward the objective door. Look overhead for the green skeleton grappling point and throw your shield at it to reach the pickup.

3. High platform — Ruby
In the next room, break the marked walls and collect the Gold along the way. Look up for a blue symbol and throw your shield at it, then quickly throw at the green statue to reach the high platform. Climb the wall, turn back, jump across the gap, and follow the path to the Ruby.
4. After the second organ section — 50 Gold
Continue the story and destroy the second organ section. Turn right, throw your shield at the green statue, then jump to the platform with the chest.

5. Third organ area — 7 Gold and the Komodo toy
After collecting the Codex entry near the third organ area, jump down and turn right toward the blue symbol. Open the path and collect the Gold, then drop through the hole to enter the optional stomach encounter. Clear it to complete the Swallowed Whole challenge and obtain the Komodo toy.
6. Beyond the portal — 14 Gold and a Life Sigil
Leave the previous area through the portal. Follow the wall on the right and enter the first opening. Look overhead for the blue symbol and open the hallway. Collect the Gold, then climb the wall at the end to reach the secret. The route passes through a tunnel of chomping teeth; use its switch to reverse their direction. The reward includes a Life Sigil.

Other pickups and challenges in the chapter
These are useful to collect, but they are not part of the six-secret count.
- Extra 5 Gold: after destroying the third organ, check the newly opened bridge to the organ’s left. This is an additional pickup, not another secret.
- Commander Thira Codex entry: in the large encounter area before destroying the third organ. Look on the opposite side of the organ, near the end of the encounter area beside the flowing stomach fluid.
- Armor Demonic Essence: defeat the Acolyte Leader in the first major enemy encounter and perform a Glory Kill to collect its heart. The reward is +10 maximum armor.
- Toe-To-Toe challenge: parry Cosmic Baron attacks ten times. The parries can be earned across one or multiple large encounters; the reward is a Reverent Grenade Launcher skin.
How to avoid missing a secret
Bethesda’s exploration guide explains that marked points of no return prevent the Slayer from returning to the previous area without restarting the level. Check the AutoMap before crossing one, especially if you have not completed the stomach encounter or checked the nearby secret routes. The map can show objectives, unexplored spaces, caches, and secret areas.

For environmental traversal, look for green nodes and superheated metal. Bethesda notes that the Shield Saw’s Charge and Throw abilities can push objects or operate gates outside combat. Blue and green symbols in this chapter are important route cues for reaching elevated or blocked-off areas.

A Life Sigil gives an immediate recovery after death without reloading a checkpoint. Carry capacity is limited, so collecting one does not mean you can stockpile them without limit. Gold, Rubies, and Wraithstones can be spent at Sentinel Shrines on weapon, Shield Saw, and melee upgrades.
