A single Reddit thread from September 14, 2025 describes a Vex “Agent” outfit disappearing from a Borderlands 4 account after a patch. The original poster later said the outfit came from the Ornate Order cosmetic pack and had been granted for free because of a bug, which the update appeared to remove. Other players in the same thread reported different missing items, and some said they owned the pack. Nothing in the thread confirms a wider problem, a shared cause, or an official response from Gearbox or 2K.
What the original report claimed
The thread is titled “BL4 – Cosmetics disappeared after update?” The poster wrote that a patch had been released in the hours before they posted, and that after relaunching the game, the Agent outfit for Vex was gone. Their own words were: “Head and clothes both just completely disappeared from my account after this patch.” That sentence is one player’s account, not an official incident description.

The correction in the original post
The poster later edited the post. They said they had realized the cosmetic came from the Ornate Order pack, that they had received it at no cost because of a bug, and that the recent patch removed that unintended access. This edit changes the story from “paid cosmetic lost” to “free cosmetic granted by error, then removed.” It is the most specific explanation in the thread, but it is still the poster’s own belief about cause. No patch note, developer statement, or entitlement record is quoted to back it up.
What other commenters reported
Other participants described missing cosmetics that were not the Agent outfit. Some said they had bought an edition or owned the Ornate Order pack, and reported losing content they believed they had paid for. These accounts conflict with the bug explanation in the edit, and the thread cannot show whether they are the same problem, separate problems, or misunderstandings about what each player owned.

What is not established
- Official explanation: No statement from Gearbox or 2K addressing missing cosmetics, or confirming the Ornate Order bug, appears in the thread or in material I could confirm.
- Patch version: The post refers only to “this patch.” No build number or release date beyond the September 14, 2025 posting is given.
- Platforms: The thread does not say which platform the poster or commenters played on.
- Scope: Nothing shows how many players were affected, or whether paying customers lost content.
- Resolution: The thread is more than a year old and does not show a fix, restoration, or refund.
How to read the phrasing people use
Players in this thread used two phrasings: “cosmetics disappeared after update” and “head and clothes disappeared from my account.” These are literal wording from one discussion. They are useful for understanding how players described the problem, but they are not measured search data, and they do not tell you how often the issue happened.
If you think you lost a cosmetic
- Check whether the item was ever purchased. Your platform storefront’s purchase history is the most reliable record; compare it with the in-game cosmetic list.
- Note the game version and date you first noticed the item missing, along with the platform you played on.
- Contact Gearbox or 2K support directly through their official support channels, and include the purchase record, platform, and date. Do not rely on community threads for a response.
- Be aware that a free item granted by a bug may be removed without a refund, and that purchased content should be handled through support rather than a forum post.
What this report proves and what it does not
The Vex Agent case is a credible individual report that a cosmetic disappeared after a patch, and the original poster’s correction gives a plausible, specific reason. Beyond that, the evidence is thin. It does not show that players broadly lost paid cosmetics, that the other missing items share the Ornate Order cause, or that the developers confirmed a bug. Treat it as one player’s account plus conflicting comments, not as a documented incident.

Borderlands 4 is the game discussed in the thread. No accessory, replacement, repair tool, or purchase is connected to this issue, and buying the game is not a fix for missing cosmetics.
