In 2024, researchers remotely guided a magnetic endoscope inside an anesthetized pig in Hong Kong from Zurich, about 9,300 km away. The procedure included a stomach biopsy, and clinicians remained beside the animal to insert and monitor the device. The PlayStation 3 Move Navigation Controller sent commands to the system; it was not itself a surgical robot.
What happened in the Zurich–Hong Kong demonstration?
Researchers from ETH Zurich and The Chinese University of Hong Kong reported a long-distance, teleoperated magnetic endoscopy procedure in a live animal model. The patient model was an anesthetized pig in Hong Kong, while the remote operator worked from Zurich. The team examined the pig’s stomach wall and collected tissue samples. The participating institutions described it as the first such long-distance demonstration in a live animal model.
The published case study measured a mean communication latency of 292.65 milliseconds and a maximum of 297 milliseconds during teleoperation. Those figures describe this experiment, not a universal safe limit for surgery. The case study in Advanced Intelligent Systems and CUHK’s announcement describe the procedure and setup.
What did the PlayStation controller actually do?
The remote operator used a Sony PlayStation 3 Move Navigation Controller as an input device. It sent navigation commands to ETH Zurich’s Navion magnetic navigation system. That system generated magnetic fields to steer and bend an approximately 4-mm magnetic endoscope inside the pig. The controller did not directly move an instrument like a hand, and it did not provide the imaging, magnetic actuation, or clinical safeguards.

- The remote operator watched the procedure and sent navigation commands over a WebSocket connection using the institutions’ standard internet infrastructure.
- The Navion system translated those commands into magnetic movement of the endoscope.
- The local clinical team monitored the procedure and could provide on-site support.
The controller is the memorable gaming detail, but the demonstration depended on the complete system: endoscope, magnetic navigation, video, communications, and local clinical staff. ETH Zurich’s technical announcement describes the platform and remote setup.
Was this remote surgery on a human?
No. The demonstration used an anesthetized pig, not a human patient. More precisely, it was teleoperated magnetic gastroscopy with a biopsy in a porcine model. Endoscopy guides a camera or instrument through an existing body opening; a biopsy removes a tissue sample. That is different from many operations people picture when they hear “surgery,” such as cutting, suturing, or reconstructing tissue.

It was also not a fully remote procedure in the sense of having nobody at the patient’s side. The Hong Kong team inserted and monitored the endoscope, and local clinicians were available to support the intervention. Nor was the robot autonomous: a remote human operator controlled the navigation.
What does a latency below 300 milliseconds prove?
It shows that the operator could complete this particular endoscopy task with the delay observed under the experiment’s conditions. It does not show that every operation can be performed safely with a 300-ms delay. Navigating an endoscope is not the same control problem as making a rapid, delicate maneuver during bleeding or suturing.

Delay is only one part of the network picture. Jitter, packet loss, video freezes, and recovery time can matter as much as the average. Safety also depends on the procedure, the speed of the instrument, the quality of visual feedback, whether the operator has tactile feedback, and whether a local team can take over. The study’s reported latency should therefore be read as a result for this demonstration, not as a clinical rule.
How does this fit into the history of telesurgery?
Remote robotic procedures predate this experiment. In 2001, the Lindbergh operation connected surgeons in New York with a patient in Strasbourg for a gallbladder operation. The newer Zurich–Hong Kong demonstration is notable for combining very long-distance remote control, magnetic endoscopy, an animal model, and a consumer-style controller—not for inventing telesurgery.

Research has also examined remote robotic procedures, teleproctoring, and multi-console telesurgery. Published work on the SSI Mantra system includes remote robotic surgery experiments and human clinical evaluations, but those are separate efforts using dedicated medical systems; they do not establish that the PlayStation interface in the pig experiment is approved for human surgery. The most plausible early value is extending specialist expertise to a local clinical team, rather than eliminating the need for one. Remote systems could support consultation, supervision, teleproctoring, training, and selected procedures at geographically isolated facilities. Disaster response, military medicine, and other settings where specialist travel or patient evacuation is difficult are also potential use cases.

Those are possible applications, not outcomes proven by the pig study. In the nearer term, remote guidance and supervised procedures are a more realistic model than a distant surgeon operating with no qualified clinicians beside the patient.
What must be solved before routine use?
A controlled animal demonstration cannot establish routine clinical safety, and the participating institutions’ announcements do not establish regulatory approval for this use. A deployable system would need evidence for specific human procedures and plans for what happens when something goes wrong.
- Network reliability: Redundant communications and tested behavior for connection loss, latency spikes, video failure, and control handoff.
- Local emergency capability: A trained team able to manage anesthesia and complications, take over, or convert to conventional care.
- Clinical validation: Procedure-specific human evidence, a medically certified platform, trained users, and defined workflows.
- Security: Authenticated control, encryption, network segmentation, monitoring, emergency isolation, and tested fallback modes. A WebSocket connection in an experiment alone does not demonstrate hospital-grade cybersecurity.
- Legal and operational rules: Licensing across jurisdictions, malpractice responsibility, credentialing, regulation, reimbursement, sterile equipment, maintenance, and insurance.
- Human factors: Testing precision, fatigue, accidental inputs, training time, and performance under stress. A controller that feels familiar to a gamer is not automatically suitable for clinical work.
The consumer controller may be inexpensive, but the overall system still requires medical robotics, imaging, operating-room infrastructure, network redundancy, training, staff, maintenance, and regulatory compliance.
So, is this the future of medicine?
Remote medical robotics is a credible part of medicine’s future, especially for specialist support and carefully selected procedures with local backup. This experiment is a meaningful proof of concept for remote endoscopy. It is not evidence that hospitals are ready for routine human operations using game controllers, or that a surgeon can safely operate from anywhere without a team beside the patient.
