Joel learns that the Fireflies’ operation intended to use Ellie’s immunity would kill her. He rescues her, then tells her the Fireflies had found other immune people and abandoned the search for a cure. When Ellie asks him to swear that this is true, he does. She answers, “Okay.” The game leaves unresolved whether she believes him.
What happens at the hospital?
Joel and Ellie reach the Fireflies’ hospital, where Joel discovers Ellie is being prepared for an operation intended to use her immunity. The procedure would not leave her alive. He takes her from the hospital, killing Fireflies in the process, and carries her away.
Joel’s choice saves Ellie, but it prevents the Fireflies from carrying out a procedure they believed could lead to a cure. The game does not show that a cure would certainly have resulted; it presents the choice through the conflict between the possibility the Fireflies saw and Ellie’s life.
What does Joel tell Ellie?
After the rescue, Joel tells Ellie that the Fireflies had found other immune people and stopped looking for a cure. That account conceals what happened at the hospital and gives Ellie a reason to believe their journey no longer has a purpose. What does Ellie mean by “Okay”?
In the final scene, Ellie asks Joel to swear that what he told her is true. Joel says, “I swear.” Ellie replies, “Okay,” and the game ends.
The game does not confirm whether Ellie believes him, recognizes that he is lying, or has made any decision about forgiving him. Writer and creative director Neil Druckmann said the word could carry different subtext for players, even though the actors had a specific interpretation in mind. Her expression and delivery invite readings, but none is established as the definitive answer.

Why does Joel lie?
Joel’s lie follows a decision made out of attachment to Ellie: he chooses to save her rather than allow the Fireflies to proceed with the operation. The ending also leaves the cost of that choice in view. Ellie had needed the journey to mean something, while Joel’s story denies that the Fireflies’ plan—and her immunity—could have made a difference. The game does not resolve the outbreak or show the consequences of a cure; it closes on the trust between the two characters.

This is the ending of the original The Last of Us, released in 2013. The Last of Us Part I is PlayStation’s faithful, ground-up remake of that single-player story, not a separate ending.
