How Do Hockey Officials Decide Whether a Knee-on-Knee Hit Deserves a Major Penalty?

A knee-on-knee collision is not automatically a major. The applicable rulebook sets the threshold, and NHL, IIHF and USA Hockey wording differs.

A knee-on-knee collision is not automatically a major penalty. Officials apply the rulebook for the competition: NHL Rule 50 bases the choice between a minor and major on the severity of the kneeing, while the IIHF and USA Hockey rules expressly tie a major to reckless endangerment. The precise wording and penalty progression differ by governing body.

What officials are judging

The rules describe kneeing as an action by a player who leads with the knee or, in some cases, extends the leg to make contact. That matters because two players’ knees colliding does not, by itself, establish that one player committed kneeing under the rule.

The decision is a rules judgment about the action and its seriousness, not a formula based only on where the players’ knees met. An injury, on its own, is not identified in the provisions below as an automatic major trigger. The applicable threshold depends on the competition.

How the major threshold differs by competition

NHL, 2025-26, Rule 50 Kneeing is leading with the knee and, in some cases, extending the leg outward to contact an opponent. A minor or major may be assessed based on severity. A major is accompanied by a game misconduct. A match penalty may be assessed if the referee judges that the player attempted to or deliberately injured the opponent by kneeing.
IIHF, 2025/26, Rule 50 A minor may be assessed for kneeing. A major plus automatic game misconduct may be assessed when the referee judges that the player recklessly endangered the opponent and a minor would not sufficiently sanction the player. Supplementary discipline may also be applied by the proper authorities.
USA Hockey, 2025-29, Rule 628 A minor plus misconduct is assessed for kneeing. A major plus game misconduct is assessed when kneeing recklessly endangers an opponent. A match penalty for reckless endangerment may also be assessed.

What the IIHF says officials should consider

IIHF Rule 50.3 identifies four considerations for judging reckless endangerment: “the severity of the infraction, severity of the contact, the degree of violence and the general reprehensibility involved.” Those are considerations, not a numerical test or a guarantee that any one factor decides the call.

The NHL and USA Hockey provisions cited here do not use that same four-factor wording. Do not treat the IIHF list as a universal checklist for every league.

How to assess a disputed call

  1. Identify the competition and season. Use its rulebook, rather than assuming NHL, IIHF and USA Hockey rules are interchangeable.
  2. Describe the action visible in the play. State what the player did with the knee or leg and how contact occurred; distinguish those observations from conclusions about intent or danger.
  3. Apply that rule’s actual threshold. For the NHL, Rule 50 allows a minor or major based on severity. For the IIHF and USA Hockey, explain the reckless-endangerment language and the respective penalty progression.
  4. Separate the rule from your video interpretation. The cited rule text does not establish that a particular approach direction, leg angle, intent finding, or injury automatically makes a play a major in every competition.

Without the footage and the relevant competition’s ruling, a specific play cannot be classified reliably from the fact that it was knee-to-knee or that a player was injured.

Use the governing body’s rulebook for the season and competition in question. USA Hockey also maintains a Rule Book & Resources page. Supplementary discipline, when applicable, is separate from the on-ice penalty decision.
