When Should a Baseball Pitcher Stop Throwing Because of a Hand Blister?

A pitcher should stop when a blister hurts, becomes red or uncomfortable, or starts changing their grip or mechanics. Learn when to seek care and what MLB’s bandage rule means.

Stop throwing as soon as a blister or hot spot becomes painful, uncomfortable, or red. Do not keep pitching through it, and stop if you are changing your grip or mechanics to protect the spot. There is no established blister-specific pitch-count or pain-score cutoff; the practical signal is that the skin is starting to hurt or affect your throw.

Stop when the blister starts to hurt or changes your throw

Repeated contact between the baseball seams and the fingers of the pitching hand can cause blisters, particularly on the index and long fingers, according to a peer-reviewed review of baseball-related blisters. If the spot becomes painful, uncomfortable, or red, end the session rather than testing whether more friction will make it worse. The American Academy of Dermatology advises stopping the activity when pain, discomfort, or redness develops, and the American Orthopaedic Society for Sports Medicine (AOSSM) says youth players should never throw with pain.

A blister that is not yet painful can still affect a pitcher’s grip. If you find yourself shifting your fingers, gripping differently, or changing your delivery to avoid rubbing it, stop for the day. That is a cautious application of the symptom-based guidance, not a validated blister-specific threshold.

Use these symptoms to decide what to do

  • End the throwing session: the area is red, painful, or uncomfortable, or the pitcher feels rubbing at the blister or starts compensating with grip or mechanics.
  • Stop and seek medical advice: there is pus or other concerning drainage, spreading redness or warmth, significant swelling, marked or worsening pain, fever, loss of sensation, uncontrolled bleeding, or persistent hand dysfunction. MedlinePlus lists infection warning signs such as pus, warmth, swelling, redness, and pain; AOSSM advises evaluation for persistent symptoms, sensory loss, dysfunction, or uncontrolled bleeding.

AOSSM also notes that reduced velocity or accuracy can signal arm fatigue and may be a reason to stop throwing for the day. That is general throwing guidance, not a blister-specific test.

Care for the blister after you stop

  1. Reduce friction. Do not resume the activity that caused the blister while it is still being aggravated.
  2. Keep it clean and covered. A clean, loose covering or blister bandage can protect the skin during healing off the field.
  3. Usually leave the blister roof intact. The AAD and MedlinePlus both advise against routinely popping or draining blisters. Their guidance differs somewhat on exceptions: the AAD says drainage may be considered for a large, very painful blister, while MedlinePlus also mentions blisters likely to pop. If drainage is being considered, ask a health professional rather than treating it as routine home care.
  4. Watch for infection or worsening symptoms. Contact a clinician if warning signs appear or the hand remains painful or impaired.

The AAD says typical blisters heal in one to two weeks. That is general skin-care guidance, not a pitcher-specific recovery schedule or clearance date.

Return to throwing without guessing at a timetable

The available guidance does not set a pitcher-specific rest duration or validated return-to-throwing test. As a cautious practical threshold, wait until gripping and throwing can be done without pain or compensatory mechanics. If symptoms persist or the blister recurs, seek assessment from a clinician or sports-medicine professional rather than repeatedly testing it on the mound.

Check league rules before wearing a bandage while pitching

Care advice to cover a blister does not necessarily mean a pitcher may wear that covering during a game. MLB’s official rule glossary says a pitcher may not pitch with an attachment—such as a bandage, tape, or bracelet—on either hand or wrist. That is an MLB rule; youth, school, amateur, and other professional leagues may have different rules, so check the rules that apply to your competition.

Why there is no blister-specific pitch limit

The cited medical and baseball guidance uses symptoms and function, not a number of pitches or a blister-size cutoff. The 2016 review reports historical MLB and minor-league blister-related disabled-list cases, but those figures are not current incidence estimates and do not define when an individual pitcher should stop. For an on-field decision, pain, redness, discomfort, and changes to grip or mechanics are more useful than trying to apply an unsupported numerical threshold.
