Prevent pitcher’s-hand blisters by reducing friction at the fingers that meet the seams, keeping nails smooth and calluses even, and addressing damp skin. If a blister forms, protect it and keep it clean rather than routinely popping it. Game-day treatment is a separate question: MLB bars pitchers from pitching with a bandage or tape attached to either hand or wrist, and rules differ by league.
Why pitchers get blisters—and where they form
Repeated gripping and throwing can make finger skin rub against baseball seams. Professional baseball athletic trainers identify the middle finger as the most common site, followed by the index finger and, less often, the thumb. A 2016 review describes the tips of the index and long fingers as frequent locations. These descriptions reflect reported patterns, not a guarantee about where an individual pitcher will blister.
Damp or clammy skin can increase friction. Jagged nail edges and uneven calluses can also contribute. The pitcher-specific guidance comes from athletic trainers and a review of MLB pitcher blisters; neither establishes in controlled comparisons that one prevention method works better than another.
How to reduce the risk
Keep nails smooth and calluses even
- Inspect the fingertips and nail edges regularly, especially where the fingers contact the seams.
- Keep nails at a normal length and file rough edges smooth and rounded. Avoid jagged corners that can catch or rub.
- Check calluses often and keep them even. The professional-trainer guidance recommends maintenance, but does not give a do-it-yourself shaving protocol; avoid aggressive cutting or filing.

Manage moisture and respond early
The PBATS article says approved antiperspirants and drying agents can reduce moisture buildup. Treat these as team- or clinician-approved options, not permission to apply an arbitrary substance to your hand or the ball. A product that changes grip or leaves residue can raise separate performance and rule concerns.
At the first localized pain, redness, or discomfort, stop and inspect the area instead of continuing to increase friction. The American Academy of Dermatology (AAD) also advises stopping the activity when pain or redness appears in a friction-prone area. Pads, gels, or sprays may be considered for recurrent friction in general sports settings, but whether any material may be worn on a pitching hand during a game depends on the competition rules.

What to do when a blister appears
For a small, intact blister
Keep it clean, leave the skin roof intact, and cover it loosely with a clean bandage. Avoid routine popping or draining: the overlying skin helps protect against infection. Stop the activity that caused the friction while the blister heals.
If it opens or drains
Wash the area with soap and water, apply petroleum jelly, and cover it. Do not peel away the remaining roof of skin. AAD says most blisters heal on their own in one to two weeks, but that general estimate is not a return-to-pitching timetable.

When to get medical help
Contact a health-care provider for pus, spreading or increasing redness, warmth, swelling, marked or worsening pain, or fever. Seek advice for recurrent or unexplained blisters, and get medical guidance sooner if you have diabetes or circulation problems. MedlinePlus lists these as reasons to contact a provider in its blister-care guidance.
A painful blister that affects grip or throwing deserves assessment by a clinician or athletic trainer. A 2025 PBATS article describes professional in-game care that can include sterile drainage and surgical glue, with infection-control steps afterward. That is team medical practice, not a home drainage tutorial or a reason to drain a blister yourself.

Can you tape a blister during a game?
Do not assume an off-field dressing is allowed in competition. MLB’s “Doctoring the Baseball” rule glossary says a pitcher may not pitch with an attachment such as a bandage, tape, or bracelet on either hand or wrist.
The 2023–24 NCAA rulebook says a pitcher must not use a bandage or other distracting item on the pitching hand, wrist, or fingers. Its note permits an athletic trainer to use a substance to stop bleeding if it dries sufficiently before play resumes. That is edition-specific language: confirm the current rule and the umpire’s interpretation for your level rather than applying MLB wording to every league.

In MLB, the same glossary addresses drying the pitching hand after mouth contact before touching the ball or pitching rubber. Follow the applicable rulebook and officials’ directions about any treatment or substance used during play.
When is it safe to pitch again?
The available medical and baseball guidance does not establish a universal number of rest days for pitchers. AAD’s one-to-two-week estimate is for ordinary blister healing, not a clearance standard. Readiness depends on whether the skin has healed, whether pain remains, and whether the pitcher can grip and throw without aggravating the area. Ask a clinician or athletic trainer to assess an open wound, infection concern, persistent pain, or compromised grip rather than trying to pitch through it.
What the published injury counts do—and do not—show
A 2016 review by McNamara, Ensell, and Farley reported that six MLB players accounted for seven blister-related disabled-list stints since 2010, totaling 151 days on the disabled list; it also reported eight minor-league players spending time on the disabled list because of blisters since 2012. These are historical counts from that review, not current incidence rates or estimates of a pitcher’s personal risk. The reviewed sources do not provide a broader current rate.
