NBA teams publicly track back injuries through league injury reports and game availability, but those reports do not show a player’s full diagnosis, rehabilitation plan, or medical clearance criteria. A recent study of NBA spine injuries can describe patterns in missed games; it cannot predict how long a particular player will be out. For a specific player, the public record is an availability tracker—not a clinical account.
How the NBA publicly tracks a player’s back injury
Under the NBA’s 2025-26 injury report policy, teams must identify a specific injury, illness, other medical condition, or other reason when a player’s participation may be affected, and submit a participation status. The usual submission deadline is 5 p.m. local time on the day before a game. For the second game of a back-to-back, the deadline is 1 p.m. local time on game day. Game-day reporting windows also apply, and reports can update throughout the day.
That system lets fans and researchers follow reported descriptions and availability designations, then compare them with whether the player appeared in a game. A designation such as “questionable” is a reporting status, not a measurement of spinal healing or a medical clearance decision. The policy is for the 2025-26 season; deadlines should be checked against the applicable season’s policy.
What NBA injury data says about missed games
A 2026 retrospective study by Gill and colleagues examined publicly reported spine injuries during the 2021-22, 2022-23, and 2023-24 seasons, including playoffs. The authors analyzed official injury reports, cross-checked public news and team communications, and counted games between injury and return. Their findings describe the cases in that sample, not treatment rules or forecasts for an individual player. Read the study in the Orthopaedic Journal of Sports Medicine.

- The study recorded 285 spine injuries among 217 athletes.
- Lumbar and lower-back regions accounted for 56.49% of reported spine-injury regions.
- Soreness (26.83%), spasm (23.48%), tightness (12.80%), and contusion (11.59%) together made up nearly three quarters of reported spine injuries.
- Nonsurgical management was reported in 98.78% of cases; four cases, or 1.22%, required surgery.
- For common public labels, median games missed were generally about one to two. Some categories had wide ranges, and uncommon diagnoses were associated with longer absences.
These are sample-level descriptive statistics. The study notes limits in public reporting, including cases with unspecified regions and substantial variation in some categories. “Games missed” records an outcome; it does not reveal what treatment a player received, how rehabilitation was scheduled, or why a team chose a return date. A median for a common label is not a reliable forecast for the next player reported with that label.
Why a public injury label cannot tell you when a player is ready
Labels such as soreness, tightness, spasm, or strain are useful for tracking what a team reported, but they do not disclose the complete clinical picture. Public reports do not establish the examination findings, imaging decisions, rehabilitation dose, symptom response, or clearance reasoning for an individual player. Nor do they show whether two players with the same label have the same underlying problem.

A team-level example shows why labels need to be read carefully: the New Orleans Pelicans’ 2025-26 media guide lists separate 2024-25 missed-game entries for Jordan Hawkins involving low-back tightness, strain, soreness, and spasm, as well as distinct entries for a lumbar annular fissure. Those entries illustrate changes or differences in public descriptions; they are not a basis for merging them into one diagnosis.
What can be said about a return to play
The NBA materials cited here do not publish a universal back-injury clearance checklist or a standard back-specific sequence used by every team. General sports-medicine guidance offers a useful framework, but should not be mistaken for a documented NBA back protocol. A 2016 consensus statement describes return to sport as a continuum, with progressive loading, scheduled review, communication among decision-makers, and attention to context. Its phrasing is: “Return to sport should be viewed as a continuum, paralleled with recovery and rehabilitation.” Read the consensus statement in the British Journal of Sports Medicine.

In practical terms, that framework is not a promise that every player follows the same stages, nor does it specify the tests or thresholds a team uses for a back injury. Which clinical checks are chosen and how a team weighs them are not established by the public injury report or the general consensus statement.

The NBA’s published concussion policy describes monitored, increasing-exertion stages and concussion-specific clearance requirements, including stationary-bike work, jogging, agility exercises, and non-contact drills. How to assess a player’s reported absence responsibly
- Check the latest league report. Read the reported description and availability status, and note when the report was updated. A status can change as the next game approaches.
- Separate the label from the diagnosis. Treat the team’s wording as the public description, not a complete medical record or proof of a particular injury mechanism.
- Track actual games played. Compare the report history with game participation, while accounting for the schedule and whether games were regular-season or playoff games.
- Use study statistics only as context. Ask how many cases fall in the category and how wide the observed range is; averages or medians from other players do not predict an individual return.
- Prefer direct team or league statements for specifics. Distinguish an official update from an inference based on a short injury-report label.
