A suspected wrist fracture is assessed with an examination and usually X-rays. If the first X-ray is normal or unclear but the symptoms and examination still raise concern, a clinician may arrange further imaging. Treatment depends on the break’s position and stability: options can include a splint or cast, realignment, or surgery.
What a wrist fracture can involve
“Wrist fracture” often refers to a break in the distal radius—the end of the radius, the forearm bone on the thumb side, near the wrist. Other bones in the wrist can fracture too. The American Academy of Orthopaedic Surgeons (AAOS) describes distal radius fractures as a common type of broken wrist in its 2024 plain-language summary.
How clinicians assess a possible fracture
Symptoms and examination
Pain and tenderness, swelling, bruising, or a visible change in the wrist’s shape can prompt evaluation. A clinician examines the wrist and considers the injury and symptoms; these signs alone cannot confirm a fracture.
X-rays and further imaging
X-rays are used to assess the bones, and several views are commonly taken. The American College of Radiology (ACR) identifies radiography as the initial imaging for acute hand and wrist trauma. A typical three-view wrist examination includes posteroanterior, lateral, and 45-degree semipronated oblique views; a two-view examination may not show enough detail. A normal or unclear first X-ray does not always rule out a fracture. Distal radius and scaphoid fractures can be difficult to see on initial radiographs. If the examination and symptoms remain concerning, the clinician may consider additional X-ray views, CT, or MRI. Whether more imaging is appropriate depends on the individual assessment.
When to seek urgent medical advice
Seek prompt medical help for severe wrist pain, inability to move the wrist or hold things, a wrist that has changed shape or colour, or loss of feeling in part or all of the hand. How treatment is chosen
The choice depends on what the images show, including the fracture’s position and stability, and on individual factors such as overall health, age, and activity demands—for example, work and hobbies. No single factor, including age or activity alone, determines the right treatment.
Splint or cast
A fracture that has not shifted may be treated with immobilization, such as a cast or a clinician-directed removable splint. For an undisplaced distal radius fracture, Cambridge University Hospitals NHS Foundation Trust describes support in a removable splint or temporary cast, with six weeks as an example for that specific care pathway; it is not a universal duration for every wrist fracture. Reduction
If bone fragments have shifted, a clinician may realign them in a procedure called reduction. The treating team uses the fracture’s features and imaging to decide whether this is needed and how to manage the wrist afterward.
Surgery
Some fractures need surgery, depending on their alignment and other features. A broken wrist does not automatically mean an operation: treatment decisions are individualized, and there is no single surgery threshold that applies to every patient.

Follow-up and recovery
Keep any prescribed cast or splint in place and follow the treating team’s advice about review and recovery. Because treatment and immobilization depend on the specific fracture, do not use a general timeline to change or stop the plan. If symptoms remain concerning after an initially normal or unclear X-ray, contact the clinician for reassessment rather than treating that first result as definitive.
What an online guide cannot determine
This guide cannot establish whether a particular wrist is fractured, interpret an individual X-ray, or select a treatment. A clinician needs to assess the injury and decide whether imaging, follow-up, or a particular treatment is appropriate.
