If an NFL team’s backup quarterback is injured, the team can use a designated emergency third quarterback only if its first two quarterbacks on the game-day Active List cannot participate because of injury or disqualification. The coach cannot bring in the emergency quarterback simply because a quarterback is playing poorly or has been removed for conduct. If either of the first two quarterbacks is medically cleared to return, the emergency quarterback must leave the game.
What happens first when the backup is injured?
Medical attention comes first. An official can call an injury timeout so the player can leave for evaluation. NFL ATC spotters also monitor the field and broadcast video and, in specified situations involving a player who appears disoriented or unstable, can prompt a stoppage for medical assessment. Team medical staff evaluate the player and make return-to-play decisions under league protocols.
An injury stoppage does not itself activate an emergency quarterback. That option depends on the team’s pregame designation and whether its first two game-day quarterbacks are unavailable for a qualifying reason.
When can the emergency third quarterback play?
NFL.com’s 2023 report reproduces the bylaw language: a club may designate one emergency third quarterback from its 53-player Active/Inactive List, excluding elevated players. The player can be activated during the game if the club’s first two quarterbacks on its game-day Active List cannot participate because of injury or disqualification. The report says clubs establish that Active List 90 minutes before kickoff.

The same bylaw language says activation cannot result from a head coach’s in-game decision to remove a quarterback for performance or conduct. The emergency designation is therefore not an ordinary depth-chart promotion that a coach can use whenever desired.

The 2023 report identifies the 53-player Active/Inactive List as the pool for the emergency designation and says elevated players are not eligible. These are details from that report’s reproduction of the bylaw, not a newly issued 2026 text.

What happens if a starting quarterback is cleared to return?
If one of the first two game-day quarterbacks is medically cleared to return after the emergency quarterback has entered, the emergency player must be removed. Under the bylaw language reproduced in the 2023 report, that player cannot continue at quarterback or switch to another position. The report says he may re-enter at quarterback if another emergency-third-quarterback situation arises.

The NFL Football Operations site presents a 2026 NFL Rulebook page. Its listed current-season rule changes concern kickoff adjustments and consultation on potential disqualifications; it does not list an emergency-third-quarterback change. That absence does not establish that the detailed bylaw text has been republished for 2026: the exact passage described above appears in NFL.com’s 2023 report.

For a suspected concussion, separate NFL guidance requires removal for assessment when signs or symptoms are present or an authorized person initiates the protocol. A diagnosed concussion requires the prescribed return-to-participation clearances before the player can return to practice or a game. This concussion procedure is specific to suspected or diagnosed concussion; it should not be treated as the procedure for every injury.
