Mafia: The Old Country has 51 trophies in the PS5 list reported by PowerPyx: one platinum, two gold, 10 silver, and 38 bronze, with no online trophies. The guide estimates 15–20 hours for platinum and rates the difficulty 4/10. Its key warning: Salvatore’s Apprentice can be missed in Chapter 3, while the True Soldato trophy requires every chapter to be completed on Hard. Plan the platinum run
Start on Hard if you want one story run
True Soldato requires completing every chapter on Hard without lowering the difficulty. Finishing the final chapter on Hard after playing earlier chapters on another setting does not meet the requirement. Hard is available from the start, so PowerPyx recommends choosing it at the beginning if you want to avoid a second full run. Protect the Chapter 3 missable
Salvatore’s Apprentice is the one missable trophy identified by the guide. In Chapter 3 at the Citrus Farm, open the safe without first picking up the note containing its combination. PowerPyx warns that carrying all safe-combination notes into chapter replay can prevent the trophy from unlocking, so do this correctly during the initial opportunity rather than relying on cleanup. Leave most cleanup for later
Chapter replay and Free-Roam are available for most remaining cleanup. Collectible progress saves immediately and carries between story and Free Ride, according to PowerPyx; some story-only items can also be revisited with chapter select. The guide recommends collecting story-exclusive items when encountered, then using Free-Roam and replay for what remains. Collectible guide.

- The Don — Complete all trophies. The guide says DLC is not required for platinum.
- The Carusu — Enzo escaped the mine; after the Prologue.
- Guest of Honor — Enzo found a new home; after Chapter 1.
- Forza San Celeste — Enzo won the Palio; after Chapter 2.
- The Family Business — Enzo learned the ropes; after Chapter 3.
- A Trip to the Coast — Enzo saved Isabella; after Chapter 4.
- This Thing of Ours — Enzo got made; after Chapter 5.
- Lira for Lira — Enzo freed the counterfeiter; after Chapter 6.
- Everything’s Golden — Enzo won the race; after Chapter 7.
- The Rat — Enzo and Cesare killed L’Ombra; after Chapter 8.
- Total Shutdown — Enzo found the foreman; after Chapter 9.

Chapter-specific and combat trophies
- Fantasma — In Chapter 5, infiltrate Ludovici’s villa without being seen. The guide says knockouts from behind are allowed; if spotted, restart from a checkpoint.
- Silent Hunter — In Chapter 13, kill Mazzone without alerting his men.
- Trail of Destruction — Eliminate 10 cars while escaping the Tonnara in Chapter 11.
- Garden Rendezvous — Meet Isabella in secret in Chapter 3 by following the optional garden objective.
- Full Steam Ahead — Beat the train in the Chapter 7 race. The guide advises holding acceleration and restarting the checkpoint after a crash.
- The Old Ways — Reach the foreman’s office in Chapter 9 without alerting enemies.
- Man of Honor — In Chapter 5, refuse Tino’s order to kill Ludovici by deliberately missing the shot; the guide says this causes a game over and unlocks the trophy.
- Bombas Away! — Kill 10 enemies with explosives.
- Cold-Blooded — Perform 30 headshots.
- Secret Assassin — Kill 20 enemies with knife takedowns; ordinary knife attacks do not count as the stealth takedowns described by the guide.
- Rare Weapons Expert: Praecisione — Kill 20 enemies with a Praecisione.
- Rare Weapons Expert: Vendetti — Kill 20 enemies with a Vendetti.
- Rare Weapons Expert: Modello — Kill 20 enemies with a Modello C96.
- Rare Weapons Expert: Repeater — Kill 20 enemies with a Repeater.
- Live by the Blade — Perform the required special knife actions. The guide notes the description can mislead: the Scannaturi and Rasolu knife classes have the actions needed.
Collectibles, equipment, and miscellaneous trophies
- Mystery Fox Domination — Find all 50 Mystery Foxes.
- The Collector — Complete the collection: 50 Mystery Foxes, 23 Photographs, 20 Saints’ Cards, and 50 Newspapers. The foxes are required in addition to the items shown in the Collection menu.
- Vulpi Misteriusa — Find a Mystery Fox.
- Bona Furtuna — Find a Trinacria coin; one is enough, according to the guide.
- Protected — Equip a Charm.
- Riding in Style — Customize a horse.
- Driving in Style — Customize a car.
- Good as New — Fix a broken vehicle using the crank.
- Fastest Man in Sicily — The guide lists this as a Chapter 7 trophy; consult its individual entry for the exact condition.
- Worthy Opponent — Listed as a Chapter 14 trophy; consult the individual entry for the exact condition.
- Not So Fast — Listed as a Chapter 13 trophy; consult the individual entry for the exact condition.
Collectibles and post-story cleanup
PowerPyx counts 143 collectible locations and divides them into story-exclusive items and Free Ride/Free-Roam items. The minimap shows most items when nearby, but Mystery Foxes do not appear there until collected. Progress saves immediately and carries between story and Free Ride; some story-only collectibles are available again through chapter select. Collectible guide.

For The Collector, track all four categories: 50 Mystery Foxes, 23 Photographs, 20 Saints’ Cards, and 50 Newspapers. Completing only the items surfaced in the Collection menu is not sufficient if the Mystery Foxes are missing. PowerPyx’s collectible guide provides location coverage.
Combat and weapon-kill requirements can be farmed by checkpoint restart, with Chapter 11 suggested by the roadmap as a convenient point for weapon types. Treat that as the guide’s route advice, not a requirement to complete them there. PowerPyx roadmap.
