Ghost of Tsushima contains graphic samurai violence, brief partial nudity, and profanity. In the United States, the ESRB rates it Mature 17+ for Blood and Gore, Intense Violence, Language, and Partial Nudity. The most significant concern for families is the violence.
How graphic is the violence?
The ESRB’s rating summary describes sword, bow, and poison-dart combat, including bloody dismemberment and decapitation. Stealth kills include neck stabbing and impalement. Cutscenes depict people being burned, including bound civilians burned at the stake, as well as beheadings; one scene shows a decapitated head held toward the screen. This is substantial, graphic violence rather than occasional blood.

The ESRB summary identifies these kinds of content but is not a complete scene-by-scene inventory, and it does not establish how often each event occurs.
Does it include nudity or sex scenes?
The cited ESRB summary describes a man’s exposed buttocks while bathing in a hot spring. The game has the Partial Nudity descriptor, which the ESRB defines as “Brief and/or mild depictions of nudity.” The cited rating materials do not describe an explicit sex scene or sexual violence.

What age rating applies?
For the United States, the ESRB assigns the original PS4 game a Mature 17+ rating, with Blood and Gore, Intense Violence, Language, and Partial Nudity descriptors. The ESRB’s Mature category is generally for ages 17 and up and may include intense violence, blood and gore, sexual content, and/or strong language. This is a US rating, not a universal age recommendation; other regions use their own rating systems.

The ESRB lists the Director’s Cut for Windows PC, PS4, and PS5 with the same main descriptors and materially similar content summary. PlayStation’s US product page lists the Director’s Cut for PS4 and PS5 and repeats the descriptors. Check the rating for the edition and region you intend to buy.

If your main concern is graphic violence, the ESRB description gives a clear reason for caution: it includes bloody executions, dismemberment, and disturbing scenes of people being burned. The nudity described is brief, and the rating summary identifies a specific profanity but does not give a frequency. Whether those elements are acceptable is a family decision, but the game’s Mature 17+ rating and violence descriptors are the strongest suitability signals.
