No. Nightmare Cows are not high enough level to drop a Monarch base. The Monarch’s quality level is 72, while the highest Nightmare Cow monster level cited is 67.
Why Nightmare Cows cannot drop a Monarch
The Arreat Summit classifies the Monarch as an elite shield and lists its quality level (qlvl) as 72 (Blizzard Entertainment’s Arreat Summit: Elite Shields). A monster must be high enough level for the base item to be eligible to drop. The cited Nightmare Cow levels do not reach that threshold.

A detailed community explanation of Nightmare Cow Level gives the area level as 64, with ordinary monsters at level 64, champions at 66, and unique monsters and their minions at 67 (d2jsp Forums, “Quick Monarch Question,” October 2010). A separate drop explorer lists the Nightmare Cow King at level 67 (D2R Drop Calculator). Even the highest of those cited levels is five levels below the Monarch’s qlvl.
Quality level and item level are different
The level 72 figure is the Monarch’s quality level, not the item level (ilvl) of a dropped shield. Quality level describes the base item’s eligibility to drop; the monster that drops an item determines its ilvl. In Nightmare and Hell, ordinary monsters take the level of the area where they appear, according to the D2R drop explorer. The specific Cow Level breakdown above comes from the 2010 community explanation, not a new in-game test.

Some forum replies use “ilvl” when discussing the Monarch’s threshold or otherwise confuse the two terms. A Blizzard D2R forum thread asking whether a Monarch can drop on Nightmare difficulty contains conflicting early replies, followed by a correction that Nightmare Cows cannot drop one (Blizzard D2R Forums, “Monarch Shield on Nightmare difficulty?”, December 2021). The distinction between qlvl and ilvl resolves the confusion.
