Yes—if “first run” means your first overall playthrough. You must first reach a Neutral ending, then reload your qualifying save and continue to the True Pacifist ending. You cannot get True Pacifist before completing Neutral, but you do not need to start a second playthrough or reset a save that meets the route requirements.
What “first run” means in Undertale
True Pacifist is gated behind completing a Neutral ending. That means even if you spare every monster on your first playthrough, you still see Neutral before the game opens the final True Pacifist sequence. After the ending, reload your save to continue. So the answer depends on what you mean by “first”: it is possible on your first overall playthrough, but not before you have completed Neutral once.
Requirements for a qualifying save
- Do not kill monsters or gain EXP; the route keeps you at LV 1.
- Befriend Papyrus, Undyne, and Alphys by completing their friendship events.
- Complete the Neutral ending, reload the qualifying save, and explore the True Lab.
You can still fight during encounters if the monster survives and you gain no EXP. Use nonlethal options such as Spare, Flee, battle-ending ACTs, or items. The route reference advises reloading a prior save if you accidentally kill a monster; if you have gained EXP/LOVE, it advises resetting instead.

First-playthrough route, in order
- Keep the run nonlethal. Resolve encounters without killing monsters or gaining EXP, and check that your protagonist remains at LV 1.
- After the Papyrus fight, visit his house. Complete his date or hangout.
- Give Undyne water, then visit her. When she collapses after the chase, take water from the cooler and pour it on her. Later, return to her house and complete her hangout.
- Finish the story through Neutral. Once the ending is complete, reload your save rather than resetting if it still qualifies.
- Complete the Alphys sequence. Depending on your progress, return through Hotland/CORE and finish the Mettaton EX encounter peacefully. Receive Undyne’s letter, deliver it under the door of Alphys’s lab, and complete the Alphys date.
- Explore the True Lab. Return to Alphys’s lab, find its four keys, restore the generator, and follow the route to the ending.
Some steps can happen earlier or be skipped depending on prior progress. For example, the route guide notes that if Omega Flowey was defeated on a previous run and Mettaton EX is defeated in the current run, Undyne may give you her letter immediately after her hangout. When you need to reload or reset
- Neutral is complete and your save still qualifies: reload it to continue toward True Pacifist. A full restart is not required.
- You killed a monster: the route guide says you can reload a save from before the kill.
- You gained EXP/LOVE: the route guide advises resetting the save.
- You failed to pour water on Undyne: the route guide also advises resetting the save.
What the True Pacifist ending changes
True Pacifist breaks the barrier so the monsters can leave the Underground, and it includes the complete ending credits. The game’s Endings reference documents the distinction between Neutral and True Pacifist.
