Usually, no. You can continue an existing Baldur’s Gate 3 campaign to receive game updates, but an update will not rewind your story or guarantee access to a scene whose trigger you have already passed. If your save can still reach the scene and meets its story conditions, keep playing; if not, you may need an earlier save—or, if you have none, a new playthrough for that specific scene.
What an update can—and cannot—do
Larian’s patches added scenes, epilogue material and character reactivity, and also fixed specific scene-flow and save-related issues. Those changes are not the same as resetting campaign progress: the patch notes do not promise that every missed scene will play retroactively or that every older save qualifies for every addition.
So the key question is not whether you must restart to download the content. It is whether your current campaign is still able to reach the particular scene you want, given its progress and past choices.
Which Karlach and Astarion changes were added?
The relevant material arrived across several updates rather than in one single release.

- Karlach’s optional ending: In Community Update #24, Larian said it was expanding the epilogues and would add a new optional ending with Karlach in Patch 2. Larian’s Community Update #24 (29 August 2023).
- Epilogue and scene changes: Patch 5 added a section after the Netherbrain’s defeat, restored cinematic portions of Astarion’s Act III Ascendant scene, and fixed a black-screen issue during his endgame romance scene. The notes also say Karlach’s camp reflection on leaving Act II can trigger during the transition into Act III if needed. Larian’s Patch 5 notes (30 November 2023).
- More epilogue reactivity: Patch 6 added Karlach and Astarion reactions in God Gale’s epilogue dialogue, including the option to request a cure for their conditions, and a new version of Astarion’s kiss scene after he ascends. It also included epilogue behavior fixes after loading a save made there and other relationship-related reactivity. Larian’s Patch 6 notes (16 February 2024).
- Targeted fixes: Patch 7 addressed, among other things, a Karlach Avernus scene-flow issue and an Astarion camp-celebration romance edge case. Such fixes can resolve a particular problem; they do not establish that all missed scenes are restored for every save. Larian’s Patch 7 notes (5 September 2024).
- Later content-patch status: Larian described Patch 8 as the final major game patch to introduce new content. It still included fixes and polish involving Karlach and Astarion. Larian’s Patch 8 announcement and notes (15 April 2025).
What to do with your current save
If you have not passed the scene’s point in the story
Update the game and continue the campaign. Whether a particular scene appears still depends on reaching its point in the story and having the relevant choices or story state. Larian’s notes list specific additions and fixes, but do not provide a complete checklist of romance flags or trigger requirements, so no single save can be guaranteed to qualify from the patch notes alone.
If you think you already passed the trigger
Load an earlier save from before that part of the campaign, if you have one, and see whether you can reach the scene from there. A patch may repair a listed bug, but it does not replay a scene automatically just because the game has been updated.

If you have no earlier save
A new playthrough may be necessary for that particular scene if your current campaign has passed its trigger and cannot reach it. You do not need to restart merely to receive patches or their generally available additions.
