Friends can play multiplayer together, including local split-screen on Xbox 360 and PlayStation 3, but that does not let a second player join the campaign.
Can you play the Call of Duty 4 campaign co-op?
No. The original Call of Duty 4: Modern Warfare campaign is for one player. Activision’s PC manual labels the mode “Single Player,” describes starting and resuming the campaign, and identifies Arcade Mode as a replay mode unlocked by completing the single-player campaign. The console manuals also distinguish the campaign from multiplayer.
That means a second player cannot join you to play through the story in co-op. Split-screen and online play refer to multiplayer matches, not a shared campaign.
What multiplayer can you play with friends?
Xbox 360
The Xbox 360 manual documents multiplayer over System Link, Xbox LIVE, and split-screen. It says split-screen multiplayer supports up to four players.

PlayStation 3
The PlayStation 3 manual documents online, LAN, and split-screen multiplayer. It says up to four players may join a split-screen match.
PC
The PC manual describes single-player and multiplayer as separate modes. It does not establish PC split-screen support, so do not assume you can play locally on one PC with multiple players based on that documentation.

Which version are you asking about?
The original Call of Duty 4: Modern Warfare and Call of Duty: Modern Warfare Remastered are separate editions. Activision’s Remastered page lists the remaster as released on November 4, 2016, for Xbox One, PlayStation 4, and PC, with a campaign and multiplayer. The page does not establish that its campaign supports co-op, so its multiplayer description should not be taken as proof of a cooperative story mode.
Check the platform and edition on your copy: original Xbox 360, PS3, and PC releases are not interchangeable with the remaster, and the documented multiplayer options differ by platform.
