In Ninja Theory’s Hellblade: Senua’s Sacrifice, Senua’s journey is physically real, but the player sees its world through her perspective. Creative director Tameem Antoniades described the game as a story-led action adventure shaped by research and consultation about psychosis, while insisting that its ending is not a cure narrative.
Is the world of Hellblade real, or a metaphor?
In a 2017 interview with GamesBeat, Antoniades answered that Senua is physically traveling: “I would say that her journey is real, as in she is physically traveling.” He also said players experience only her reality. The game does not step outside her point of view to certify which visions are objectively happening and which are shaped by her experience.
That distinction lets the game join a physical journey to a subjective, fantastical world without reducing the whole adventure to a dream or a metaphor. Antoniades compared the relationship between reality and fantasy to Don Quixote mistaking a windmill for a giant. In his account, the imagery has roots in a real place, even as Senua’s perception transforms what the player sees. GamesBeat’s interview is explicitly marked as containing narrative spoilers.
How did Ninja Theory research psychosis for the game?
Antoniades said the team understood that depicting psychosis was difficult and could provoke backlash if handled carelessly. GamesBeat reports that Ninja Theory consulted mental-health experts and people with lived experience; an Unreal Engine developer interview also describes collaboration with neuroscientists and people who had experienced psychosis.

The interviews describe a development approach, not a clinical certification. Consultation informed the portrayal, but it does not establish that the game reproduces psychosis faithfully for every person or that playing it produces a mental-health benefit. Antoniades’s account of individual audience reactions is personal testimony, not population-level evidence.
His broader creative aim was to use games to bring players closer to another person’s experience. In a 2015 development diary, he wrote: “When playing a good game, the player enters a state of trance, losing all sense of themselves and falling into a world of visuals, sound and story that is unlike any other medium.” That is a statement of artistic ambition, not proof that a game can fully transmit someone else’s lived experience. What historical grounding did the team use?
Antoniades pointed to the Vikings’ arrival in Orkney and named Cambridge history consultant Dr. Elizabeth Ashman Rowe. The interview presents these as elements of the game’s grounding; they should not be mistaken for evidence that every historical detail or image in Senua’s world is exact. The contrast is central to the design: a historical setting filtered through a protagonist whose experience is fantastical and personal.
Is Senua cured at the end?
No. Antoniades rejected the interpretation that Senua is cured. “The important thing that some people have gotten wrong, and I don’t know why they’ve interpreted it that way, is that she’s somehow cured at the end,” he told GamesBeat. He said the voices and visions remain. His explanation is that Senua lets go of the belief that she was cursed and responsible for Dillion’s death—not that her experiences disappear.

He also declined to reduce the ending’s symbolic details to one definitive answer. The distinction matters: the story offers a change in how Senua understands guilt and loss, not a neat resolution in which psychosis is simply overcome.
How did the game’s design support its story?
Antoniades described Hellblade as a story-focused action game intended to combine AAA presentation with indie sensibilities. Ninja Theory wanted to create its own intellectual property and challenge the assumption that a game between those scales would not work. The team removed conventional systems it saw as distractions from that focus:

The removal of those systems is a stated design choice, not evidence that the game has no mechanics or guidance at all. Antoniades also described building an in-house performance-capture studio, with assistance from Vicon, 3Lateral, Cubic Motion and Epic. That account describes part of the workflow, not the entirety of the production pipeline.

GamesBeat also reported a $395,000 Wellcome Trust grant; the interview does not state the grant year. That figure is GamesBeat’s reporting and should not be read as an independently verified amount.
What came later in the Senua series?
In a separate development from the 2017 interview, Xbox Wire reported on June 7, 2026 that Ninja Theory had announced a game titled Senua. Studio head Dom Matthews described it as an action-adventure expanding combat, puzzles and exploration. This announcement is later franchise context, not a statement by Antoniades or part of his account of making Hellblade. Xbox Wire’s announcement establishes the game’s description, but not current release or availability details.
